流式细胞术与PCR-SSP法检测HLA-B27的比对及HLA-B27亚型的研究

摘　　要：目的探讨流式细胞术(FCM)与序列特异性引物聚合酶链式反应(PCR-SSP)在人类白细胞表面抗原-B27(HLA-B27)检测中的应用,并观察HLA-B27亚型在本地区的分布情况。方法采用PCR-SSP和FCM检测88例疑似强直性脊柱炎(AS)患者HLA-B27。结果 88例AS疑似患者中FCM法检测HLA-B27结果阳性82例,阴性6例,阳性率为93.2%;PCR-SSP法检测阳性83例、阴性5例、阳性率为94.3%。分型结果 B*2705型41例占49.4%;B*2704型37例占44.5%;B*2702型1例占1.2%;阳性但未分辨型别4例。结论 Kappa一致性系数K=0.905,PCR-SSP法与FCM检测HLA-B27结果具有较高的一致性。本地区易感HLA-B27等位基因亚型主要以HLA-B*2704和HLA-B*2705为主。Objective To investigate the application of flow cytometry（FCM）and sequence-specific primer polymerase chain reaction（PCR-SSP）method in the detection of human leukocyte antigen-B27（HLA-B27）and to observe the distribution of HLA-B27 subtypes in the local region.Methods The HLA-B27 level in 88 cases of suspected ankylosing spondylitis（AS）was detected by FCM and PCR-SSP.Results Among 88 cases of suspected AS,82 cases were HLA-B27 positive and 6cases were HLA-B27 negative by FCM,the positive rate was 93.2%;whereas83cases were HLA-B27 positive and 5cases were HLA-B27 negative by PCR-SSP,the positive rate was 94.3%.In the subtyping results,41 cases were B＊2705genotype,accounting for 49.4%,37 cases were B＊2704genotype,accounting for 44.5%,1case was B＊2702genotype,accounting for 1.2%,4cases were positive without distinguished genotype.Conclusion The Kappa consistency coefficient is 0.905,the detection results of HLA-B27 by PCR-SSP and FCM have higher consistency.The susceptible HLA-B27 allele subtypes are mainly HLA-B＊2704and HLA-B＊2705.
