小剂量轻比重不同浓度罗哌卡因单侧腰-硬联合麻醉应用于老年患者全髋关节置换术的效果观察

摘　　要：目的观察不同浓度的小剂量轻比重罗哌卡因应用于单侧腰-硬联合麻醉在老年患者全髋关节置换术（THA）的效果。方法选择2013年8月~2015年7月收治的需行THA老年患者138例,将其随机分为A、B、C组,均选择L3-4作为穿刺节段,A组给予0.15%罗哌卡因2.5 mL,B组给予0.25%罗哌卡因2.5 mL,C组给予0.5%罗哌卡因2.5 mL,比较三组患者效果及不良反应发生率。结果 B组学流动动力学更为稳定,差异有统计学意义（P〈0.05）。B组平面消退时间、运动阻滞恢复时间均高于A组,低于C组,差异均有统计学意义（P〈0.05）;B组运动阻滞起效时间低于A组,高于C组,差异均有统计学意义（P〈0.05）。A组、B组不良反应发生率显著低于C组,差异均有统计学意义（P〈0.05）。结论0.25%罗哌卡因2.5 mL轻比重罗哌卡因应用于单侧腰-硬联合麻醉在老年患者THA效果较佳,且不良反应发生率低,具有重要临床价值。Objective To observe effect of small dose light proportion of different concentration ropivacaine lateral combined spinal-epidural anesthesia applied in the treatment of elderly patients with total hip arthroplasty.Methods In August 2013 to 2015 July from 138 patients requiring total hip arthroplasty in elderly patients,which were randomly divided into group A,group B and group C choose L3-4 as puncture segment and a group given0.15%ropivacaine 2.5 mL,group B received 0.25%ropivacaine 2.5 mL,group C treated with 0.5%Luo ropivacaine2.5 mL,compared three groups of patients the effect and adverse reaction incidence.Results B group flow dynamics is more stable,the difference was statistically significant（P〈0.05）.Group B plane regression time,motor block recovery time were higher than those in group A,lower than that in group C,with statistically significant differences（P〈0.05）;group B motor block onset time lower than group A,group was higher than that of C group,the differences were statistically significant（P〈0.05）.A group,B group,the incidence of adverse reactions was significantly lower than that in C group,the differences were statistically significant（P〈0.05）.Conclusion The application of 0.25%ropivacaine 2.5 mL of hypobaric ropivacaine in combined spinal-epidural anesthesia in elderly patients with total hip joint replacement has better effect,and adverse reactions occurred rate is lower.It has an important clinical value.
