K-L入路切开复位内固定治疗髋臼骨折伴股骨头脱位的临床疗效

摘　　要：目的研究K-L入路切开复位内固定治疗髋臼骨折伴股骨头脱位的临床疗效。方法回顾性分析2013年3月至2014年3月到湖北省孝感市中心医院行K-L入路切开复位内固定术的30例髋臼骨折伴股骨头脱位患者临床资料,采用改良Merled,Aubigne与Postel评分系统评价手术后髋关节功能恢复情况,采用Matta标准评定骨折复位效果,同时随访1年,观察术后并发症发生情况。结果末次随访时髋关节功能评分为(16.4±1.6)分,显著高于治疗前的(12.3±1.5)分,差异具有统计学意义(P<0.05)。末次随访时患者优良率为76.67%,解剖复位19例,满意复位9例,复位较差2例。术后并发症总发生率为43.33%。结论 K-L入路切开复位内固定治疗髋臼骨折伴股骨头脱位效果显著,但并发症较多,应严格操作,降低并发症发生率。Objective To study the clinical curative effect of K - L approach open reduction and internal fixation in the treatment of acetabular fractures with femoral head dislocation. Methods The clinical medical records of 30 patients with acetabular fractures with femoral head dislocation who underwent K - L approach open reduction and internal fixation in our hospital from March 2013 to March 2014 were retrospectively analyzed. The modified merled, Aubigne and Postel scoring systems were used to evaluate the postoperative recovery of hip joint function. The reduction effect of fractures was evaluated by Matta criteria. Meanwhile, with a year of follow - up, the occurrence of postoperative complications was observed. Results At the end of the last follow - up, the score of hip joint function （ 16.4 ±1.6） was significantly higher than that before the treatment （ 12. 3 ±1.5 ） （ P 〈 0.05 ）. The excellent and good rate of patients at the end of the last follow - up was 76.67%. There were 19 cases with anatomic reduction, 9 cases with satisfactory reduction and 2 cases with poor reduction. The total incidence rate of postoperative complications was 43.33%. Conclusion The effect of K - L approach open reduction and internal fixation in the treatment of acetabular fractures with femoral head dislocation is significant but the complications are more, which should be strictly operated to reduce the incidence of complications.
