摘  要:目的:应用红外热成像技术探索得气与经穴效应关系。方法:对以往国内外原发性痛经的研究方法进行分析总结,围绕本研究的目的、特点进行设计。针对本研究设计了非随机对照试验,样本量36,分为健康对照组、寒凝对照组及针刺组,其中针刺组根据不同综合干预措施又分为期望得气组(粗针,深刺、施手法)和期望不得气组(细针,浅刺、不施手法)。记录针感,并用红外热像仪观察三阴交、地机、阴陵泉、血海、关元穴点体表温度40 min。主要观察指标为经穴体表红外热像温度和腹痛视觉模拟评分(VAS)。预期结果:1寒凝PD患者经穴体表温度显著低于健康人。2针刺三阴交穴可以显著升高寒凝PD患者经穴体表温度。3针刺三阴交得气可以显著升高寒凝PD患者经穴体表温度。预期结论:人体经穴体表红外热像温度降低可能能够作为诊断寒湿凝滞证原发性痛经的辅助诊断依据之一。经穴体表红外热像温度变化可能能够作为针刺效应及得气效应的客观指标。Objective:Use infrared thermal imaging technology to observe the relationship between Deqi and the acupoint effect. Methods:We reviewed the researches about primary dysmenorrhea(PD) at home and overseas in recent 20 years, analyzed and summaried their design methods, giving our study's purpose and differences. We proposed this study protocol. It was designed as a nonrandomized controlled clinical trial, and 36 subjects were enlisted, and they were divided into four groups, the health control group, the PD with cold and dampness stagnation pattern control group, the Deqi group and no Deqi group. For the control group, we just observed the skin temperature of acupiont Sanyinjiao(SP6), Diji ( SP8 ), Yinglingquan( SP8 ), Xuehai ( SP10 ) and Guanyuan (CV4) through the thermal infrared imager for 40min, without needling. For both treatment groups, we observed the same acupionts as the control group , while needling SP6 bilaterally, but they were performed with different stimulations . The patients in Deqi group were needled with diameter O. 30 mm × length 40 mm filiform needle and perpendicularly needled for 30 mm × 35 mm with equal reinforcing and reducing ma- nipulation (evenly twirling and rotating for 30 seconds at instant needling and 10 min and 20 rain later after needling ). The no Deqi group was performed with diameter 0.18 mm x length 13 mm filiform needle and subcutaneously needled without needle manipulation. Needles were retained for 30 min.
