摘  要:目的分析听神经瘤术中影响面神经电生理监测判断的相关因素。方法收集161例听神经瘤手术病例,依据术中电生理监测结果与术后面神经恢复是否一致分为一致组(n=101)和不一致组(n=60),分析不同的影响因素对术中行电生理监测判断面神经保留情况的影响。结果两组性别、术中血压、是否有其他慢性病史、术中呼气末二氧化碳分压(PETCO2)、肿瘤切除情况的差异具有统计学意义(P<0.05)。依据Logistic分析结果,术中血压情况和术中PETCO2情况是术中电生理监测保护面神经的影响因素(P<0.001)。结论术中血压和PETCO2情况影响术中电生理监测对保护面神经结果的判断。Objective To explore the influential factors of the facial nerve neurophysiological monitoring during the operation for vestibular schwannomas. Methods A total of 161 patients with vestibular schwannomas undergoing surgery were included. The patients were divided into consistency group (n = 101) and non-consistency group (n = 60) according to the consistence between intraoperative neurophysiological monitoring results and postoperative functional recovery of the facial nerve. The influence of different factors was analyzed during the process of neurophysioI6gical monitoring diagnosing the preservation of facial nerve. Results There were significant differences in gender, intraoperative blood pressure, other chronic diseases, intraoperative end-tidal carbon dioxide pressure (PETCO2), tumor resection between the two groups (P 〈 0.05). Logistic analysis results showed that intraoperative blood pressure and intraoperative PETCO2 were the influential factors on the intraoperative neurophysiological monitoring diagnosing the facial nerve preservation (P 〈 0.001). Conclusion Intraoperative blood pressure and intraoperative PETCO2 have influence on the judgement of intraoperative neurophysiological monitoring for facial nerve preservation results.
