强直性脊柱炎全髋关节置换术的围术期处理及手术时机探讨

摘　　要：目的分析强直性脊柱炎合并髋关节强直患者实施全髋关节置换术的临床治疗效果。方法 34例(55髋)强直性脊柱炎患者,均实施全髋关节置换术,观察其临床效果。结果术前髋关节总活动度(36.2±8.7)°,术后髋关节总活动度(193.2±12.5)°,检查术后均无感染、脱位发生,Harris评分术前、术后分别为(32.0±8.0)、(85.0±7.2)分,髋关节功能优良率为87.3%,经X线查无假体松动移位,异位骨化5髋(9.1%)。结论强直性脊柱炎患者采用全髋关节置换术治疗改善患者病变,效果显著。Objective To analyze clinical effect by total hip replacement for patients of ankylosing spondylitis complicated with hip joint ankylosis. Methods A total of 34 （55 hips） patients with ankylosing spondylitis received total hip replacement. Their clinical effects were observed. Results Total range of hip motion was （36.2 ＋ 8.7）° before operation and （193.2 ＋ 12.5）° after operation. Examination showed no occurrence of infection and dislocation. Preoperative and postoperative Harris scores were respectively （32.0 ＋ 8.0） and （85.0 ＋ 7.2） points. Good rate of hip joint function was 87.3%. X-ray examination showed no prostheses loosening or displacement. There were 5 hips with heterotopic ossification （9.1%）. Conclusion Implement of total hip replacement in ankylosing spondylitis patients can improve their lesion with remarkable effect.
