摘 要:目的分析2012年云南省楚雄克山病病区人群硒营养水平。方法以氢化物原子荧光光谱法测定楚雄6个在20世纪80年代有不同克山病病情自然村的大米和儿童发硒含量,利用Spss软件统计分析数据。结果研究区大米平均硒含量为0.020±0.003 mg/kg(n=179),属典型克山病病区粮食硒缺乏水平;儿童发硒平均为0.274±0.058 mg/kg(n=179),总体已达中等硒营养水平,但仍有27.9%的儿童发硒含量处于缺乏的边缘水平,且女生人数多于男生,男生发硒平均含量为0.286±0.073 mg/kg(n=90),女生发硒平均含量为0.261±0.034 mg/kg(n=89),差异有统计学意义(t=2.977,P<0.05)。结论云南省楚雄克山病病区大米硒含量和儿童发硒含量均按罗平关(非病村)→新地(轻病村)→红豆树(重病村)和龙屋庙(重病村)顺序依次降低,与1984年一致;楚雄地区粮食硒含量相比1984年无明显变化,但儿童发硒含量增加了一倍;当地仍有相当比例人群处于硒营养缺乏的边缘状态,急需采取措施增加硒摄入量,预防克山病。Objective To analyze selenium(Se) nutritional state of children in Keshan disease area of Chuxiong, Yunnan in2012. Methods The Se content in children's hair and rice from their families were analyzed with hydride generation-atomic fluorescence spectrometry, using Spss software for statistical analysis. Results The Se concentration in rice was 0.020 ±0.003 mg/kg on average(n=179), showing the typical Se deficiency level of Keshan disease area. But the average Se concentration in children's hair was 0.274±0.058 mg/kg(n=179), an adequate level in terms of Se nutrition and doubled the level in1984. Even so there were still 27.9% of the children in marginal Se deficiency states, more in girls than in boys. Average content of hair selenium was 0.286±0.073 mg/kg(n=90) in the boys and 0.261±0.034 mg/kg(n=89) in the girls, with statistical difference(t=2.977, P〈0.05). Conclusions Both the Se concentration in rice and children's hair decreased among four villages in the order of non-disease, lightly affected, and severely affected by Keshan disease, the same as in the survey in 1984.The Se content of rice showed no significant change since 1984, but the Se content of children's hair had doubled, there was still a quarter of children in marginal Se deficiency, especially in girls. Effective measures should be taken to improve Se intake level in the area to prevent Kashan disease.
