摘　　要：目的探讨类风湿关节炎(RA)患者血清中同型半胱氨酸(Hcy)的水平及其在合并心血管病变(CVD)的RA患者血清中的变化。方法选取于2011年-2014年在河北省人民医院就诊的143例RA患者及143例健康体检者,根据患者是否患有心血管病变分为RA合并CVD组及RA未合并CVD组,用酶循环法法分别检测3组的Hcy水平,同时记录RA患者的病程、DAS28评分、C-反应蛋白(CRP)、类风湿因子(RF)、抗环瓜氨酸肽抗体(ACCP)、血沉(ESR)、血脂等相关指标,分析Hcy与各项指标之间的相关性。结果 RA患者血清Hcy水平明显高于健康体检者,差异具有统计学意义(P<0.05)。RA合并CVD患者血清Hcy水平明显高于RA未合并CVD患者,差异具有统计学意义(P<0.05)。相关性分析显示,RA患者血清Hcy水平与其关节肿痛数、ESR、LDL-C呈显著正相关(P<0.05)。结论 RA患者血清Hcy水平较健康体检者有所升高,并与心血管疾病的发展密切相关。Objective To investigate the levels of Hcy in the serum of rheumatoid arthritis （RA） patients and the changes in RA combined with cardiovascular disease（CVD）. Methods 143 cases of rheumatoid arthritis patients and 143 healthy adults in Hebei General Hospital were selected during 2011 - 2014. According to whether patients suffering from cardiovascular disease, they were divided into RA with CVD group and RA without CVD group. The levels of homocysteine（Hcy） in three groups were detected with circulating enzymatic method. At the same time, course of RA patients, DAS28 score, C - reactive protein （CRP）, rheumatoid factor（RF）, anti cyclic citrullinated peptide antibody （ACCP）, erythrocyte sedimentation rate （ESR）, blood lipid and other related indicators were also recorded. Then the correlations between the Hey and the indicators were analyzed. Results The levels of serum Hcy in patients with RA were significantly higher than those in healthy subjects, and the difference was statistically significant （ P 〈 0.05）. The level of serum Hcy in patients with RA combined with CVD was significantly higher than that in patients with RA without CVD, and the difference was statistically significant （ P 〈 0.05 ）. The levels of Hcy were positively correlated with the number of swollen joints, ESR and LDL - C in RA patients （ P 〈 0.05 ）. Conclusion The Hcy levels of RA patients increased compared with healthy adults, and are closely related with the development of cardiovascular disease.
