股骨近端锁定钢板治疗头下型股骨颈骨折

摘　　要：目的：评估股骨近端锁定钢板治疗头下型股骨颈骨折的临床疗效。方法：选择上海交通大学医学院附属同仁医院2013年6月—2014年6月收治的头下型股骨颈骨折患者15例,均采用股骨近端锁定钢板内固定治疗。术后规律随访,通过X线检查了解骨折愈合情况,采用Harris评分评定髋关节功能,评估手术效果。结果：15例头下型股骨颈骨折患者平均随访16.7个月（12-24个月）,骨折均愈合良好,无伤口感染;1例出现股骨头部分坏死,但无关节活动障碍;未出现骨折延迟愈合及骨不连情况,无内固定松动,无脱钉、断钉,无股骨头旋转、短缩等并发症。术后髋关节Harris评分提示13例优良,优良率为86.67%。结论：头下型股骨颈骨折采用股骨近端锁定钢板固定,骨折复位满意,固定牢靠,术后恢复较理想,骨折愈合率较高。Objective ：To assess the clinical efficacy of proximal femoral locking plate for the treatment of subcapital femoral neck fracture .Methods ：A total of 15 patients with the subcapital femoral neck fracture who were admitted and treated with proximal femoral locking plate in Tong Ren Hospital ,Shanghai Jiao Tong University School of Medicine during Jun .2013 and Jun .2014 were enrolled .Regular postoperative follow-up was done .Fracture healing was assessed by X-ray and function of hip joint was evaluated with Harris score .Results ：All the 15 patients were followed up for 12-24 months （mean 16 .7 months） . All the cases were well healed and there was no wound infection .Partial necrosis of the femoral head was found in 1 case , however there was no joint function limitation .The delayed union of fracture or bone nonunion was not found ,and there was no internal fixation loosening ,nail off ,broken nail ,neck rotation ,or neck shortening .The Harris score indicated that the excellent and good rate was 86 .67% （13/15） .Conclusions ：Proximal femoral locking plate provides good fracture reduction , stable fixation ,good postoperative recovery ,high healing rate for the treatment of subcapital femoral neck fracture .
