摘  要:目的研究睾丸微石症(testicular microlithiasis,1M)患者精液质量及精子DNA损伤程度。方法TM组:1346名男性不育患者,采用高频彩色多普勒超声检查双侧睾丸,其中35名确诊为TM,平均年龄(23.39±3.36)岁;对照组:23名正常生育力男性,平均年龄(25.28±5.18)岁。所有研究对象均留取精液标本进行精液常规分析;采用抗活性半胱氨酸蛋白酶3(Caspase.3)标记精子,使用流式细胞仪检测精子细胞凋亡率;精子染色质扩散实验分析精子DNA碎片率;苯胺蓝染色法评价精子核成熟度。所有数据均进行统计学分析。结果TM组与对照组的精子细胞凋亡率分别为(0.86±0.25)%VS(0.19±0.12)%,精子核成熟度分别是(56.7±8.5)VS(11.6±3.3),精子DNA碎片率分别为(31.6±4.6)vs(13.3±3.9),以上指标两组间比较差异具有统计学意义(P〈0.01)。TM组与对照组的精子浓度、精子总活力、前向运动精子比率以及正常形态率比较差异也均具有统计学意义CP〈0.05)。结论TM可能会导致精子质量下降,精子DNA损伤程度增加。Objeotive To investigate the semen quality and the sperm DNA damage in patients with testicular mierolithiasis (TM). Methods Total of 1346 patients with male infertility underwent ultrasound examination for the bilateral testicles, and 35 of them were diagnosed as TM, with an average of 23.39±3.36 years old. Twenty-three male patients with normal fertility were recruited as the controls, with an average of 25.28±5.18 years old. All the subjects underwent semen analysis.The sperms were tagged with Caspase-3, and their apoptosis were measured by flow cytometry, the sperm DNA fragmentation rate was measured by the sperm chromatin diffusion test. Toluidine blue staining method was used to evaluate the sperm maturity. All the data were analyzed by SPSS soRware. Results The sperm apoptosis rate were 0.86±0.25% VS 0.19±0.12% in the TM group and the control group, respectively. The sperm maturity rate and DNA fragmentation rate were 56.7±8.5 VS11.6vs3.3 and 31.6±4.6 VS13.3±3.9, respectively, with significant difference (P〈0.01). there were significant differences in the mobility, total viability, progressive motive rate and normal morphology rate between TM group and the control group (P〈0.05). Conolusion TM may result in a decrease in sperm quality and an increase in sperm DNA damage.
