青年高血压患者视觉非随意注意的事件相关电位研究

摘　　要：目的研究青年高血压对视觉非随意注意的影响。方法采用事件相关电位(ERP)成分P3a,对24例原发性青年高血压患者和23例正常对照者的视觉非随意注意进行客观评价,并对所有受试者采用简易智能精神状态检查(MMSE)量表进行认知功能测评。结果与正常对照组(29.00±0.74)相比,高血压患者MMSE评分明显降低(25.92±1.25,t=-10.36,P=0.01),且高血压组P3a波幅显著降低[(4.19±0.62)μV,F=4.88,P=0.03],其中以顶区波幅下降更明显(F=10.23,P=0.003);高血压组与正常对照组间P3a潜伏期差异无统计学意义(F=2.01,P=0.16);高血压与正常对照组间N2波幅差异无统计学意义(F=0.12,P=0.73);视觉P3a波幅与MMSE评分无显著正相关性(r=0.19,P=0.21)。结论青年期高血压病导致视觉非随意注意加工损害,事件相关电位成分P3a可作为早期评估青年高血压病患者视觉非随意注意的客观证据。Objective To investigate the effects of adolescent hypertension on visual involuntary attention. Methods This study used P3a component of the event related potentials （ERP） to evaluate visual involuntary attention of 24 essential adolescent hypertensives and 23 normotensives, and all the participants were submitted to cognitive assessment by mini-mental-state-examination（MMSE）scale. Results Compared with control group （29. 00 ± 0. 74）, the MMSE score of patients with hypertension was significantly decreased （25. 92± 1.25 ,t = - 10. 36,P = 0. 01 ）, and the amplitude of P3a of adolescent hypertensive patients was significantly decreased [ （4. 19 ± 0. 62）pN, F = 4. 88, P = 0. 03 ], particularly at PZ sites （F = 10. 23, P = 0. 003）. There was no significant difference in P3a latency between hypertension group and normal control group（F =2. 01 ,P =0. 16）. There was no significant difference in the amplitude of N2 between hypertension group and normal control group（ F = 0. 12, P = 0.73 ）. There was no significant positive correlation between the amplitude of P3a and MMSE score （ r = 0. 19, P = 0. 21 ）. Conclusion The adolescent hypertension causes visual involuntary attention impairment. The component of event related potentials of P3a can be early used to value the visual involuntary attention objectively in adolescent hypertensive patients.
