摘　　要：目的了解住院病人艾滋病病毒（HIV）的感染状况,为有效控制医源性感染提供依据。方法对2012-2014年在重庆医科大学附属第一医院的186 919例住院病人,用酶联免疫吸附试验（ELISA）和胶体金法检测HIV抗原及抗体,可疑标本送疾病预防控制中心做确证试验。对确证阳性的病人进行人口学特征、科室分布等综合分析。结果 2012-2014年,共检测186 919例病人,HIV确证阳性率0.31%（583例）。583例HIV阳性者分布于医院29个临床科室,其中男性458例,女性125例,男女比例3.7：1;20~69岁年龄段占86.62%（505例）,已婚者占66.38%（387例）。结论 HIV感染者分布广泛。建议医院对高危住院病人进行HIV筛查,明确病人健康状况,杜绝医源性感染,保护医患双方利益。Objective To study HIV（Human immunodeficiency virus,HIV） infection status among the inpatients and provide basis to control the iatrogenic infection effectively.Methods Inpatients from 2012 to 2014 in our hospital received HIV antigen and antibody testing with the method of enzyme linked immunosorbent assays（ELISA）and colloidal gold method.Those screened as HIV positive got confirmatory test by the Center for Disease Control and Prevention.Then the HIV positive inpatients were analyzed for their demographic characteristics and inpatient distribution of clinical departments.Results 583（3.1‰） out of 186 919 inpatients were found infected with HIV,with 505 cases（86.62%） between 20 and 69 years old and the male/female ratio 3.7：1（458/125）.Most of them were married,accounting for 66.38%（387/583）.The 583 HIV infected persons were distributed in29 out of 38 clinical departments in the hospital.Conclusion The rate of HIV infection is rising year by year and the distribution of infected persons is becoming wider.It is suggested that inpatients should receive HIV test,esp.for those with high risk behaviors.This will help know the status of HIV infection among the inpatients,to avoid potential nosocomial infection and possible medical disputes,thus to protect the interest of both doctors and patients.
