摘 要:目的了解胰岛素诱导基因蛋白2(INSIG2)-102G〉A多态性、饮食运动行为与儿童肥胖指标的关系,为阐明儿童肥胖的病因及制定预防控制策略提供依据。方法选择北京市海淀区5所学校1 093名7~18岁中小学生作为研究对象,进行身体测量和饮食运动行为问卷调查。用基质支持的激光释放/电离飞行时间质谱分析(MALDI-TOF MS)方法检测-102G〉A多态性。在显性模型下,采用多元线性回归分析-102G〉A多态性与儿童肥胖性状的关系,分层分析不同饮食、运动水平下-102G〉A多态性与儿童肥胖指标的关系,并检验多元线性回归中的交互作用。结果 -102G〉A多态性与经常饮用碳酸饮料对体质量指数(BMI)有交互作用(P=0.033)。在经常饮用碳酸饮料的儿童中,-102G〉A多态性A等位基因与BMI升高存在相关性(β=1.27,95%CI=0.03~2.52,P=0.045),而不经常饮用碳酸饮料的人群中无此相关性。分性别分析,仅在男生中发现经常饮用碳酸饮料者的-102G〉A多态性与BMI存在相关性(β=2.06,95%CI=0.36~3.75,P=0.018),腰围和臀围的分析均发现类似的结果;在每天体育锻炼时间〈1 h的男生人群中发现,-102G〉A多态性A等位基因与臀围升高有相关性(β=4.71,95%CI=0.07~9.34,P=0.046)。结论 INSIG2基因-102G〉A多态性与经常饮用碳酸饮料、每天体育锻炼时间的交互作用对儿童BMI、腰围、臀围的影响存在性别差异。Objective To explore the -102G〉A polymorphism in insulin induced gene 2 and its relationship with child- hood obesity, diet and physical activity. Methods A total of 1 093 children aged 7 to 18 years were selected from 5 schools in Haidian district, Beijing. Physical examination and questionnaire investigation were performed for all subjects. Genotyping of - 102G〉A was performed with matrix assisted laser desorption ionization time of flight mass spectrometry ( MALDI-TOF MS). The re- lationship between childhood obesity and the -102G〉A polymorphism were analyzed by different levels of physical activity and diet behavior by using the dominant genetic model. Results Interactive effect of the -102G〉A polymorphism and carbonated beverage on BMI (P=0. 033) was found. The -102G〉A polymorphism was associated with BMI in subjects who drank carbonated beverage daily (β= 1.27, 95% CI=0. 03-2.52, P=0. 045). While similar result was not found among people who did not often drink car- bonated beverage. In analysis stratified by sex, association between -102G〉A and BMI only in boys who drank carbonated beverage everyday was found(β=2.06, 95% CI=0. 36 -3.75, P=0.018). Similar effect on waist and hip circumstance was found. There were similar trends in people who had physical activity less than 1 hour/d(β=4.71, 95% CI=0. 07 -9.34, P=0.046). Conclu- sion The association of the -102G〉A polymorphism with BMI, waist and hip circumstance is more significant among children who prefer carbonated beverage and less physical activity.
