摘  要:目的探讨乙型肝炎肝硬化(LC)患者血清iNKT细胞数量、活化标志物CD38和HLA-DR表达水平与肝损伤程度的关系。方法采用流式细胞仪检测LC患者外周血iNKT细胞频率及CD38和HLA-DR分子在iNKT细胞上的表达水平,分析其与LC患者肝功能Child-Pugh分级、MELD评分及临床肝功能指标相关性。两组数据比较采用MannWhitney U检验,相关性分析采用Spearman rank检验。结果与健康对照组相比,LC患者组iNKT细胞频率为0.106%,显著降低(P=0.026),CD4^+iNKT细胞比例为66.7%,显著升高(P<0.01),CD4-iNKT细胞比例为31.2%,显著降低(P<0.01);CD38和HLA-DR在iNKT、CD4^+iNKT和CD4-iNKT细胞上表达水平分别为14.2%、24.9%,18.8%、19.1%,18.4%、36.9%,明显升高(P<0.05),随着Child-Pugh分级有逐渐升高的趋势,但分级之间差异无统计学意义。CD4^+iNKT细胞上CD38和HAL-DR分子表达水平与ALB呈显著负相关(r=-0.496,P=0.019;r=-0.501,P=0.018)。结论 LC患者iNKT细胞频率明显降低;活化水平显著升高,且与肝功能损害程度相关,提示iNKT细胞可能参与LC肝损伤。Objective To investigate potential correlation between frequency of peripheral blood invariant nature killer T(iNKT)cell,expression of activation markers CD38 or HLA-DR and severity of hepatic injury in patients with hepatitis B virus(HBV)-related liver cirrhosis(LC).Methods Frequency of peripheral blood iNKT cells and expression of CD38 or HLA-DR molecules were detected by flow cytometry to analyze their correlations with Child-Pugh score,model of end-stage liver disease(MELD)score and serum biochemical parameters for liver function in HBV-related LC patients.Comparison and correlation analysis between the two groups were performed by Mann-Whitney U test and Spearman rank test,respectively.Results When compared to healthy controls,LC patients displayed significantly lower frequency of iNKT cells and proportion of CD4-iNKT cells(0.106%,P=0.026;31.2%,P〈0.01),while higher proportion of CD4^+iNKT cells(66.7%,P〈0.01),higher expression of CD38 and HLA-DR molecules on iNKT,CD4^+iNKT and CD4-iNKT cells(14.2%,24.9%,18.8%,19.1%,18.4%,36.9% respectively,P〈0.05),respectively.However there was no statistically different expression of CD38 and HLA-DR among different Child-Pugh score.Meanwhile,expression of CD38 and HLA-DR on CD4^+iNKT cells was negatively correlated with albumin level(r=-0.496,P=0.019;r=-0.501,P=0.018).Conclusion Compared to those in healthy controls,peripheral blood iNKT cells in LC patients manifested significantly lower frequency,but higher cellular activation level which was associated with aggravation of hepatic injury.In another word,iNKT cells might play a role in the pathogenesis of liver injury in LC patients.
