痔结扎与缝扎术后并发出血的对照研究

摘　　要：为对比观察痔的单纯结扎术和“8”字贯穿缝扎术术后并发出血的发生情况，明确2种术式术后的疗效差异，将160例Ⅱ～Ⅳ期内痔住院患者随机分为结扎组和缝扎组各80例，分别进行单纯结扎术和“8”字贯穿缝扎术治疗。术后给予同样处理，观察9个不同时间点创面出血情况，运用四级三分评分判定标准统计，统计住院时间、创面愈合时间。结果显示，缝扎组术后4h、24h出血评分均低于结扎组（P〈0．01），缝扎组术后12h、第4天、第5天、丝线脱落当天、丝线脱落第2天出血评分低于结扎组（P〈0．05）；2组术后48h、第3天出血评分差异无统计学意义（P〉0．05）。2组患者住院时间、创面愈合时间差异无统计学意义（P〉0．05）。结果表明，以出血为主的痔病，选择“8”字贯穿缝扎术，止血效果更好。Through observing and comparing the complicated bleeding status after a procedure of alone ligation and a procedure of ＂8＂-like penetrating suture 8〈 ligation to find out the difference between the 2 procedures,this study retrospectively divided the 160 admitted patients with Ⅱ～Ⅳ stage of internal hemorrhoids into ligation group and suture ＆ ligation one,80 cases for each,which above two groups respectively received alone ligation procedure and ＂8＂-like suture ＆ ligation procedure, and all postoperative same treatment;observed both groups wound-surface bleeding status at 9 different time-points, made the statistics on hospitalization time, wound-surface healing time by using ＂four classes and three points＂ judge criteria.As results,at 4,24 h after operation,the score rating on bleeding status of suture ＆ ligation group was very significantly lower than that of ligation alone group（ P 〈0.01） ;at 12 h,on the 4th and 5th day,in the day and next day ligature fall off of the score rating on bleeding status was significantly lower than that of ligation alone group（ P 〈0.05） ;but at 48 h,on the 3rd day after operation there was no statistical difference between both groups（ P 〉0.05） ,and in hospitalization time and wound-surface healing time so was yet（ P 〉0.05）.Results show that for hemorrhoids predominanted by bleeding it is better to choose ＂8＂-like penetrating suture ＆ ligation which may obtain more satisfactory stopping bleeding efficacy.
