摘  要:目的探讨应用超纯透析液对维持性血液透析(MHD)患者炎症状态及生活质量评分的影响。方法选择我院MHD患者52例,观察其在使用超纯透析液试验前、试验后(6个月)血清肿瘤坏死因子(TNF)-a、白细胞介素(IL)-1β、ILlRa、IL-6、C反应蛋白(CRP)等指标的变化,同时评估患者健康相关生活质量评分的变化。结果使用超纯透析液6个月后MHD患者的血清TNF—a、IL-1β、IL-1Ra、IL-6的检测水平较使用前有明显的下降趋势(P〈0.05);SF-36(sF-36)评分在精力、精神健康及心理健康方面的评分有升高趋势(P〈0.05)。结论超纯透析液能够部分改善MHD患者的炎症水平和健康相关生活质量评分。Objective To explore the effects of ultrapure dialysate on the inflammatory status and quality of life in maintenance hemodialysis patients (MHD). Methods 52 patients were selected from the hemodialysis center, Beijing Boai Hospital, China Rehabilitation Research Center. The study was a prospective, self-controlled study, patients were followed up for 6 months. High-flux dialyzers were used before and after the study. Ultrapure dialysate concentrate was used instead of standard dialysis concen- trate in the study. Predialysis blood and dialysate were collected in 0 months and 6 months. Tumor necrosis factor-a(TNF-u) ,interleukin-lβ(IL-1β),interleukin-lRa(IL-IRa),interleukin-6(IL-6) and C reactive protein(CRP) were detected. The short form-36 health survey(SF-36) was used to assess the quality of life in MHD patients in 0 months and 6 months. Results The levels of TNF-u, IL-1β, IL-1Ra and IL-6 in 6 months were declined significantly compared with 0 months after using ultrapure dialysate ( P 〈 0. 05 ). There were substantial improvement in vitality( VT), mental health(MH) and mental component summary measure( MCS ) in 0 months and 6 months (P 〈 0.05 ). Conclusion Ultrapure dialysate can partly reduce the inflammatory status and improve the quality of life in MHD patients.
