摘 要:背景 机械通气(mechanical ventilation,MV)作为围术期重要的呼吸支持方式和临床危重症患者救治时辅助通气重要的一种手段.MV所致肺损伤(ventilator induced lung injury,VILI)是最常见也是最严重的并发症之一.高迁移率族蛋白(high mobility group protein,HMG)B1是人和哺乳动物体内普遍存在的一组高度保守的非组蛋白核蛋白,近年来其被发现作为一种关键性晚期炎症因子参与了致死性炎症反应的致病机制,从而成为国内外研究的热点.已有学者研究发现,HMGB1在MV肺损伤的机体中表达上调.目的 明确HMGB1在VILI中所起的作用及相关研究进展.内容 简要综述VILI与HMGB1在国内外的研究现状、HMGB1在VILI中的作用及其机制的最新研究.趋向 HMGB1参与了MV所致肺损伤中相关炎症通路的组成,有望成为治疗VILI的新靶点.Background Mechanical ventilation (MV) is an essential respiratory supporting method during perioperation as well as one of the most important means in saving and curing critically ill patients. MV can initiate some severe complications in lung including ventilator induced lung injury(VILI). High mobility group protein(HMG) B1, a chromatin-associated protein that involved in DNA organization and transcriptional regulation, is recently verified to be involved in lethal inflammatory response. Ding and other researchers have found that HMGB1 is up-regulated in the progress of VILI. Objective A comprehensive understanding of the role of HMGB1 in VILI and related research advances are clearly summarized. Content In this review, recent findings on the role of HMGBI and involved signaling pathways in VILI are briefly discussed. Trend HMGB1 plays an important role in VILI and is expected to be one of new target for preventing VILI.
