光动力三阶段治疗鲍温样丘疹病临床疗效观察

摘　　要：观察5-氨基酮戊酸光动力疗法(ALA-PDT)三阶段治疗鲍温样丘疹病的疗效和安全性。方法回顾性分析我科诊治的82例鲍温样丘疹病患者,分为单纯CO_2激光组21例、CO_2激光联合咪喹莫特治疗组30例、光动力三阶段治疗组31例。结果 CO_2激光组患者有效率为52.38%(11/21),复发率57.14%(12/21);CO_2激光联合咪喹莫特治疗组有效率为80%(24/30),复发率26.67%(8/30);ALA-PDT三阶段治疗组患者的有效率为83.87%(26/31),复发率为3.23%(1/31)。三组患者有效率及复发率比较差异均有统计学意义(P<0.05)。结论 CO_2联合咪喹莫特及光动力疗法治疗鲍温样丘疹病,可以显著提高有效率,减少复发,不良反应发生率低,值得临床推广。Objectives： To observe the efficacy and safety of 5- amino acid photodynamlc merapy （ALA-PDT） three stages treatment of Bowenoid papulosis. Methods： a retrospective analysis of 82 cases of Bowenoid papulosis pa- tients who have been diagnosed and treated in our department, which are divided into pure CO2 laser group （21 cases）, CO2 laser combined with imiquimod treatment group （30 cases） and ALA-PDT 3 stages treatment group （31 cases）. Results： the effective rate of the CO2 laser group was 52.38% （ 11/21 ） and the recurrence rate was 57.14% （ 12/21 ） ; the effective rate of CO2 laser combined with topical imiquimod treatment group was 80% （24/30） and the recurrent rate was 26. 67% （8/30） ; In the ALA-PDT three stages treatment group, the effective rate was 83.87% （26/31）, the recurrence rate was 3.23% （1/31）. The effective rate and recurrence rate were statistically significant different among the three groups. （P 〈0. 05 ）. Conclusions： CO2 laser combined with imiquimod and photodynamic therapy treatment for Bowenoid papulosis can significantly improve effective rate, reduce the recurrence and the ad- verse reaction rate is low, it is worthy of clinical promotion.
