摘 要:目的评价脂血标本对核酸检测结果的影响。方法收集核酸检测非反应性脂血标本40份,分别与HBV、HCV、HIV病毒灭活血清混样,根据脂血程度分3组:中度脂血组、重度脂血组和对照组;有乳糜析出的标本10份,分别与HBV、HCV、HIV病毒灭活血清混样,对乳糜层和非乳糜层进行核酸扩增检测。重度脂血组和对照组分别与极低浓度病毒灭活血清混样,比较检出率。结果中度脂血组HBVDNA、HCVRNA、HIVRNA的Ct值分别是(34.0±0.38)、(35.9±0.80)、(37.29±1.02),重度脂血组的Ct值分别为(34.0±0.39)、(36.1±0.77)、(37.32±1.19),与对照组的Ct值(33.9±0.45)、(36.0±0.75)、(37.15±0.86)比较,差异均无统计学意义(P>0.05);与极低浓度病毒灭活血清混样的重度脂血组和对照组比较,阳性检出率无显著差异。乳糜层HBV、HCV、HIV的Ct值分别是(34.11±0.37)、(36.86±0.77)、(37.36±0.78),与非乳糜层的Ct值(34.06±0.53)、(36.03±0.73)、(37.25±1.02)比较差异无统计学意义(P>0.05)。结论脂血对血液核酸检测结果无影响。Objective To evaluate the effect of lipemia samples on the nucleic acid test( NAT) results of S201 Cobas blood screening system. Methods 40 lipemia samples that were nonreactive on NAT were collected and mixed with HBV,HCV,HIV virus inactivated serum samples. According to the degree of lipemia,they were divided into three groups: moderate lipemia group and severe lipemia group and control groups. Ten samples of precipitated chyle were collected,and mixed with HBV,HCV,HIV virus inactivated serum samples. Chyle layer and the non-chyle layer were amplified for detection.The virus inactivated serum samples were mixed with very low concentrations of virus in severe lipemia group and control group respectively. The detection rates of the two groups were compared. Results The Ct values of moderate lipemia group were( 34. 0 ± 0. 38),( 35. 9 ± 0. 80),and( 37. 29 ± 1. 02) for HBVDNA,HCVRNA and HIVRNA,respectively. In severe lipemia group,the Ct values were( 34. 0 ± 0. 39),( 36. 1 ± 0. 77),and( 37. 32 ± 1. 19),respectively. No significant difference was observed when comparing with the control group( P 〉0. 05). Comparing with the control group,the positive detection rate demonstrated no significant difference between the severe lipemia group and the control group. The Ct values of Chyle layer were 34. 11 ± 0. 37,36. 86 ± 0. 77,37. 36 ± 0. 78 for HBVDNA,HCVRNA and HIVRNA,respectively,showing no significant difference with the non-chyle layer( P〉 0. 05). Conclusion There was no significant effect of lipemia samples on the NAT results.
