无针接头不同消毒方式对预防导管相关性感染的影响

摘　　要：目的：探讨无针接头不同消毒方式对预防外周中心静脉导管（PICC）相关性感染的影响。方法：选取在本院行PICC置管治疗的148例肿瘤患者为研究对象,采用随机数字表法分为观察组和对照组各74例。对照组采用传统的碘伏棉签消毒导管无针接头,观察组采用独立包装的酒精棉片消毒无针接头。结果：观察组发生静脉炎1例,导管细菌定植1例,导管相关性感染（CRI）发生率2.70%明显低于对照组14.86%（χ2=6.831,P〈0.05）。导管相关性感染病原菌以革兰氏阳性菌（G＋）为主（72.73%）,其次为革兰氏阳性菌（G-）（27.27%）。结论：采用独立包装酒精棉片包裹无针接头,运用正反揉搓法消毒,可有效降低导管相关性感染的发生率,促进患者康复。Objective： To study effect of needleless connector different disinfection methods for prevention of PICC catheter related infections. Methods： 148 tumor patients with PICC catheter therapy were divided into observation group and control group. The control group were used traditional povidone-iodine swabs sterilized needle catheter connector,observation group with individually wrapped alcohol swabs sterilized needle-free connector. Results： Observation group occurred one case of phlebitis and one case of catheter colonization. Catheter related infections（ CRI） was significantly lower in the observation group than the control group（ 2. 70%vs14. 86%,P〈 0. 05）. Catheter related infections pathogen mainly gram-positive bacteria（ G ＋）（ 72. 73%）,followed by gram-positive bacteria（ G-）（ 27. 27%）. Conclusion： Adopt ing independent packing alcohol pad package needleless connector with using positive and negative knead method of disinfection can effectively reduce the incidence of catheter related infections,promote patients recover.
