孕妇MRI护理干预与胎儿影像质量的相关性评价

摘　　要：目的：探讨孕妇MRI护理干预对胎儿影像质量效果的评价。方法：100名孕妇胎儿MRI受检对象,随机分为实验组和对照组,实验组60例,对照组40例,两组均行美国GE Discovery 3.0T MRI胎儿成像,实验组成像前行护理干预,包括MRI安全性的告知;知情同意书的签署;训练屏气;排空膀胱;仰卧位,足先进;心理反应的护理等。由二位从事多年MRI的专家以优质胎儿MRI图像质量标准进行盲法评分。结果：实验组分值为8.08±1.45,对照组分值为6.13±1.36,实验组分值与对照组具有统计学差异（t=6.77,P=0.00）;以标准6分为阈值,将总评分6分以上定义为图像质量良好,实验组56/60,对照组11/40,实验组优质图像比例明显高于对照组,差异具有统计学意义（χ^2=8.170,P=0.004）。结论：孕妇MRI检查前给予护理干预,对提高胎儿影像质量,满足诊断要求具有重要的临床价值。Objective： To explore the correlation between MRI nursing intervention on the pregnant woman and imaging quality of fetus. Methods： 100 pregnant women,who took fetus MRI examination,were at random divided into Experimental Group（ n = 60）and Contrasting Group（ n = 40）. The two groups were given MRI fetus imaging with American GE Discovery 3. 0T. Before the examination,nursing intervention was conducted to the Experimental Group,such as informing of MRI safety,singing on the Medical Informed Consent Document,training of breath-holding,bladder emptying,lying on the supine position with feet inward first,psychological response nursing,etc. Two MRI experts adopted the blind scoring according to the high-quality fetus MRI imaging criteria. Results： The mean score of the Experimental Group and the Contrast Group were 8. 08 ± 1. 45 and 6. 13 ± 1. 36 respectively and there existed statistical significance（ t = 6. 77,P = 0. 00）. With the standard mark 6 as the threshold value,the image scoring over that value was defined as the high-quality one. The rate of high-quality MRI images in the Experimental Group（ 56 /60） was higher than that of the Contrasting Group（ 11 /40） and the difference between them had statistical significance（ χ~2= 8. 170,P = 0. 004）. Conclusion： MRI nursing intervention on the pregnant woman before the examination has significant clinical value in improving the fetus imaging quality and satisfying diagnostic requirements.
