摘 要:目的探讨基于空腹血糖值分布的早期糖尿病筛查的切点。方法测量随机查体人员的空腹血糖(FPG)值与糖化血红蛋白(HbA1c)值,基于峰分离方法对查体人群的FPG分布进行统计处理,获得空腹血糖受损(IFG)人群的FPG分布特征值,根据其平均值与标准差确定FPG切点。结果查体人群的HbA1C值随FPG的增加而增加;对查体人群FPG分布统计处理发现,健康人群与IFG人群的FPG值都呈高斯分布,健康人群的FPG=5.504±0.39 mmol/L,而IFG人群的FPG=6.306±0.232 mmol/L,基于"μ-σ"确定FPG切点值定为6.074 mmol/L,与2003年指南值6.1 mmol/L一致,而基于"μ-2σ"才能有效进行IFG早期筛查。结论基于随机查体人群的FPG分布统计处理,健康人群与IFG人群的血糖值符合高斯分布,由IPG人群的FPG分布获得IFG的FPG切点5.842 mmol/L,与临床工作中监测到的血糖值吻合。Objective Investigate the critical fasting plasma glucose (FPG) for screening diabetes based on the analysis of FPG distribution. Methods Physical examination was carried out for randomly selected people and their FPG and HbAlc data were collected. The peak separation for the distribution of FPG data was statistically performed using Gauss distribution models to obtain the characteristic values for health people and IFG. The critical FPG for screening IFG was proposed based on the FPG distribution of IFG people. Results showed that the HbAlc values increase with the increase of the FPG values. It was found that the FPGs for healthy and IFG people follow the Gauss distribution based on the peak separation processing of FPG data. For healthy and IFG people, the mean FPG and its standard deviation are (5.504±0.39) mmol/L and (6.306±0.232) mmol/L, respectively. The estimation of μ-σryielded a FPG cut point of 6.074 mmol/L for IFG, being consistent with guideline value of 6.1 mmol/L published by Chinese Diabetes Society in 2003. However, the FPG cut-point by "μ-2σ"is reasonable to screen IFG. Conclusion The Gauss distribution characteristic parameters were obtained by peak separation processing of FPG data distribution for both healthy and diabetic people. Based on the Gaussian parameters for diabetes, the FPG cut-point of 5.842 mmol/L for screening early diabetes was acquired which reasonably agrees with the FPG used for clinical monitoring of IFG diabetes by authors.
