尿液联合检测对早期诊断糖尿病肾病的临床意义

摘　　要：目的通过检测糖尿病患者血清视黄醇结合蛋白(RBP)、转铁蛋白(TRF)、尿微量清蛋白(MALB)等指标,探讨尿液联合检测对早期诊断2型糖尿病肾病的临床意义。方法从本院选取早期糖尿病发生肾脏损害患者、单纯糖尿病患者、健康体检者各50例。使用实验室标准方法对上述患者的血清及尿液留样,并检测RBP、TRF、MALB、尿胱抑素C(Cys-C)、尿素(BUN)以及肌酐(CR)的水平。结果血清RBP、BUN、CR和尿TRF、MALB、Cys-C水平在糖尿病早期肾损伤组中明显高于单纯性糖尿病组和正常对照组,差异有统计学意义(P<0.05);而单纯糖尿病组血清RBP、BUN、CR及尿TRF、MALB、Cys-C的水平与正常对照组对比,差异无统计学意义(P>0.05)。RBP+TRF+MALB联合检出阳性率达到94%,高于单独检测,差异有统计学意义(P<0.01)。结论血清RBP与尿TRF、MALB联合检测比单独检测,对早期发现2型糖尿病肾脏损害更具有临床意义,具有推广价值。Objective To investigate the clinical significance of the combined detection of urine in the early diagnosis of type 2diabetic nephropathy by detecting serum retinol binding protein（ RBP）,transferrin（ TRF）,urinary microalbumin（ m Alb） of diabetic patients. Methods In our hospital,we select fifty early nephropathy patients with type 2 diabetes（ T2DM）,50 normal renal function patients with type 2 diabetes（ T2DM） as the subjects of observation,and another 50 healthy volunteer as the control group. The patient＇s serum RBP,TRF,urine MALB,Cys- C,BUN,and CR were detected by the standard method,and statistical methods were applied for statistical analysis. Results The levels of the RBP,BUN,CR in serum and the levels of TRF,MALB,Cys- C in urine were significantly higher in diabetic patients with early renal injury than those in simple diabetes group and normal control group,with the differences statistical significant（ P〈0. 05）,while the that between the simple diabetes group and the normal control group had no statistical significance（ P〈0. 05）. The positive rate of RBP ＋ TRF ＋ MALB was94%,higher than that of single detection,and the difference was statistically significant（ P〈0. 01）. Conclusion The combined detection of RBP,TRF and MALB has more clinical significance in early detection of renal damage in patients with type 2diabetes than the single detection,so it is worth promotion.
