摘  要:目的对分别发生在幼儿园和小学的2起猩红热疫情进行病原学检测和分子特征分析,为流行病学分析提供资料。方法按照国家行业标准WS 282—2008进行病原菌分离和鉴定,采用PCR法扩增链球菌致热外毒素基因spe A、spe B和spe C,参照Pulse Net发布的单核细胞增生李斯特菌的PFGE操作程序进行脉冲场凝胶电泳分析,应用PCR法扩增emm基因,进行测序,通过比对获得emm基因型。结果 2起疫情中共检出9株化脓性链球菌,血清学分群结果判定均为A群链球菌,所有分离株均携带spe B基因,spe A和spe C基因携带情况因emm基因型不同而变化,9株分离株被分成2个PFGE型别,测序比对的基因型分别为emm12和emm1。结论具有特定PFGE图谱和毒素-基因谱的分别属于emm12和emm1基因型的不同克隆的GAS菌株,引起本地区2起低年龄儿童猩红热的社区暴发。Objective Etiologic detection and molecular characteristics analysis were conducted in two outbreaks of scarlet fever occurred respectively in kindergarten and primary school,so as to provide data for epidemiological analysis. Methods Pathogens were isolated and identified according to the national industrial standard WS 282—2008,pyrogenic exotoxin genes( spe A,spe B and spe C) were amplified by PCR method. PFGE analysis was done according to Pulse Net Listeria monocytogenes PFGE protocol. Gene emm were amplified by PCR method,sequenced and blasted to get their emm genotypes. Results 9 strains of Streptococcus pyogenes were isolated from two outbreaks,proved to be group A Streptococcus by serum identification result. All isolates carried spe B gene,isolates carrying spe A and spe C gene varied with emm genotype,9 isolates were discriminated into 2PFGE types,and genes for sequencing and comparison were proved to be emm12 and emm1 genotypes respectively. Conclusion Two different clone of GAS belonging to emm12 and emm1 respectively,characterized by specific PFGE pattern and toxin- gene profile,were responsible for two community outbreaks of scarlet fever in low age children in the region.
