糖化血红蛋白联合尿微量蛋白早期诊断糖尿病肾病的价值

摘　　要：目的探讨联合检测糖化血红蛋白（Hb Alc）、尿微量蛋白（U-m Alb）在糖尿病肾病早期诊断中的临床价值。方法选择2014年1月~2014年6月确诊为2型糖尿病肾病的患者80例（观察组）和正常健康体检者80例（对照组）为研究对象,分别采用胶乳免疫聚集抑制法和快速免疫比浊法检测Hb A1c和U-m Alb水平。结果观察组与正常对照组比较,Hb Alc和U-m Alb水平均明显增高,两组间比较差异有统计学意义（P〈0.05）;U-m Alb检测诊断阳性率高于Hb Alc诊断阳性率（P〈0.05）;两项指标联合检测阳性率显著高于各指标的单项检测（P〈0.05）。结论 Hb Alc和U-m Alb联合检测可提高糖尿病肾病早期检测阳性率,对疾病的诊断和治疗有重要意义。Objective To study the significance of glycosylated hemoglobin（ Hb Alc） and urinary protein（ U-m Alb） in the diagnosis of diabetic nephropathy. Methods A total of 80 patients with typeⅡdiabetic nephropathy,80 healthy examinees were recruited from our hospital between January 2014 to June 2014. Hb Alc and U- m Alb levels were detected with latex sticky immunity repression method and turbidimetry method. Results The expression levels of Hb Alc and U- m Alb in diabetic nephropathy patients were significantly higher than healthy examinees groups（ P〈0. 05）. The U- m Alb for the diagnosis of diabetic nephropathy showed significant differences compared with the Hb Alc（ P〈0. 05）; The combined detection of these indicators is significantly better than the detection of each indicator alone（ P〈0. 05）. Conclusions Combination of Hb Alc and U- m Alb were helpful for early diagnosis and treatment of diabetic nephropathy.
