中老年脑多形性黄色瘤型星形细胞瘤的MR分析

摘　　要：目的:分析中老年脑多形性黄色瘤型星形细胞瘤(PXA)的MRI表现。方法:回顾性分析12例经手术病理证实的中老年PXA的MR资料,所有患者均行MR平扫加增强检查。结果:12例均单发且位于脑表浅部位,11例位于幕上,1例位于幕下。10例囊实性,1例囊性,1例实性。囊壁和肿瘤实性部分T1呈等或稍低信号,T2呈等或稍高信号,增强后8例实性部分明显强化,4例呈斑点、片状不均匀轻到中度强化。4例壁无或轻度强化,8例囊壁部分或全部明显强化。5例无瘤周水肿,7例轻度瘤周水肿。2例囊实性病例术后复发且发生间变。结论:发生于中老年人的脑多形性黄色瘤型星形细胞瘤的MR特征类似于年轻人群的,术前MRI平扫及增强可以做出明确诊断。Purpose： To analyze the MRI features of pleomorphic xanthoastrocytoma（PXA） in middle-aged patients. Methods： Plain and enhanced MRI data of 12 cases of pathologically confirmed PXA after surgical operation were retrospectively analyzed. Results： All the 12 cases had solitary tumor located in the superficial part of the brain, 11 cases of which located at supratentorium region and 1 case of which located at subtentorium region. Ten cases were with cystic-solid lesions, one was with cystic lesion, and one was with solid lesion. The cyst wall and solid part of the tumor was iso-intensity or hypo-intensity at T1 WI and iso-intensity or hyper-intensity at T2 WI. Solid parts of the tumors were obviously enhanced in 7 cases, and 4 cases displayed punctiform and patchy-like mild to moderate enhancement. Three cases had no or slight enhancement in cyst wall, and seven cases had partial or complete enhancement in cyst wall. Five cases had no peritumoral edema, and seven had mild peritumoral edema. Two cases were with cystic-solid lesion relapsed with anaplasia after surgery. Conclusions： MR characteristics of PXA in middle-aged patients were the same as that in young patients, and the diagnosis can be made before surgery by plain and enhanced MRI.
