血必净注射液和乌司他丁治疗慢性阻塞性肺疾病急性加重期合并全身炎症反应综合征对比研究

摘　　要：目的:探讨血必净注射液和乌司他丁治疗慢性阻塞性肺疾病急性加重期合并全身炎症反应综合征的疗效。方法:选取我院收治慢性阻塞性肺疾病急性加重期合并全身炎症反应综合征的患者73例,按随机数字表分为C组24例,A组25例,B组24例。C组给予常规治疗,A组在常规治疗基础上加血必净注射液50 m L+NS 100 m L日2次静滴,B组在常规治疗基础上加乌司他丁10万U+NS 100 m L日2次静滴,7 d为1个疗程,治疗1个疗程。观察并比较3组患者的临床疗效,治疗前、治疗第3天、治疗第7天炎症反应情况、凝血功能及细胞免疫功能水平,以及MODS发生情况。结果:治疗后,与C组相比较,A组和B组的临床疗效较高(P<0.05),A组和B组PCT降低(P<0.05),A组和B组PLT上升、Fib和D-二聚体下降(P<0.05),差异均有统计学意义。结论:血必净注射液和乌司他丁均能快速有效改善慢性阻塞性肺疾病(AECOPD)合并全身炎症反应综合征(SIRS)患者的临床症状,提高临床疗效,减轻炎症反应,改善凝血功能,其中血必净效果更为明显。Objective：To study the Xuebijing Injection and ulinastatin treatment of chronic obstructive pulmonary disease with acute aggravating period combined the curative effect of systemic inflammatory response syndrome. Methods：Selection our hospital acute exacerbation of chronic obstructive pulmonary disease period with 73 cases of patients with systemic inflammatory response syndrome,according to random number table is divided into 24 patients of group C,25 cases of group A,B group of 24 cases.Given routine therapy group C,group A on the basis of routine therapy plus Xuebijing Injection 50 m L＋ NS,100 m L,secondary static drops group B on the basis of routine therapy plus ulinastatin secondary static drop 100000 units ＋ NS 100 m L day,7 days for a period of treatment,the treatment of a course of treatment. To observe and compare the clinical efficacy of three groups of patients before treatment,3 days,7 days the inflammation,blood coagulation function and cellular immune function,and MODS. Results：After treatment,compared with group C,the clinical efficacy of higher in group A and group B（P〈0.05）,PCT lower in group A and group B（P〈0.05）,group A and group B PLT and Fib and D- dimer decreased（P〈0.05）,the difference had statistical significance. Conclusion：Xuebijing Injection and ulinastatin can quickly and effectively improve the clinical symptoms in patients with AECOPD combined SIRS,improve clinical curative effect,reduce inflammation,improve the function of blood coagulation,including Xuebijing Injection effect is more apparent.
