治疗股骨干骨折三种方法效果分析

摘　　要：探讨小切口复位交锁髓内钉内固定结合远端斜位透视锁钉技术治疗股骨干骨折临床疗效。自2010年7月～2014年7月收治应用髓内钉治疗的股骨干骨折患者96例，年龄20岁～63岁，平均40．5岁，其中男60例，女36例，按照A0分型，A型54例，B型30例，C型i2例，随机分为3组，每组32例，A组小切口复位交锁髓内钉内固定结合远端斜住透视锁钉技术，B组闭合复位交锁髓内钉内固定，C组小切口复位交锁髓内钉内固定。通过观察手术时间、术中出血量、透视次数、膝关节活动度、骨折愈合时间，进行疗效评估。手术时间、术中出血量、透视次数A组与B组、c组比较均有统计学意义（P〈0．05），膝关节活动度、骨折愈合时间A组、B组、c组组间比较均无统计学意义（P〉0．05）。小切口复位交锁髓内钉内固定结合远端斜位透视锁钉技术治疗股骨干骨折能大大缩短手术时间，减少了术中出血量及透视次数，避免了过多的医源性辐射，临床疗效满意。To explore the clinical effect of small incision reduction and interlocking intramedullary nail fixation combined with distal oblique angle locking nail in the treatment of femoral shaft fractures. From July 2010 to July 2014, 96 patients with femoral shaft fractures treated with intramedullary nailing were treated, aged 20 years - 63 years, with an average of 40.5 years, including 60 males and 36 females. According to AO classification, 54 cases were A type, 30 cases were B type, 12 cases were C type, cases were randomly divided into 3 groups, each group had 32 cases. In the A group, the small incision reduction and interlocking intramedullary nail fixation combined with distal oblique angle locking technique, the B group closed reduction and interlocking intramedullary nail fixation, C group of small incision reduction interlocking intramedullary nail fixation. The efficacy was evaluated by observing the operation time, bleeding volume, the number of X-ray, the activity of the knee joint and the healing time of the fracture. The operation time, bleeding volume, and the number of A in the patients with B were statistically significant （P〈0.05）, and there were no significant differences in the activity of knee joint, fracture healing time of A group, B group and C group （P〉 0.05）. Small incision reduction and interlocking intramedullary nail internal fixation combined with distal oblique perspective lock nail in treatment of femoral shaft fracture can greatly shorten the operation time, reduce the amount of bleeding and fluoroscopy times, avoid excessive iatrogenic radiation and clinical curative effect satisfaction.
