五草汤联合阿德福韦酯治疗乙型病毒性肝炎30例临床观察

摘　　要：目的:观察五草汤联合阿德福韦酯治疗乙型病毒性肝炎的临床疗效。方法:将60例乙型病毒性肝炎患者随机分为治疗组与对照组各30例,对照组予阿德福韦酯治疗,治疗组在对照组治疗基础上联合五草汤治疗,连续治疗1年后观察两组患者临床疗效、不同时段肝功能指标(ALT、AST、TBIL、A/G)及HBV-DNA定量等指标变化。结果:总有效率治疗组为83.3%,对照组为66.7%,组间比较,差异有统计学意义(P<0.05);两组肝功能治疗前后组内比较及治疗后组间比较,差异均有统计学意义(P<0.05);治疗第3、6个月时HBV-DNA载量组间比较无明显差异,而第9、12个月时治疗组患者HBVDNA转阴率与对照组比较,差异有统计学意义(P<0.05)。结论:五草汤联合阿德福韦酯治疗乙型病毒性肝炎具有良好临床疗效,可明显改善患者肝功能,提高乙肝病毒载量转阴率。Objective： To observe the clinical efficacy of Wucao decoction combined with adefovir dipivoxil in the treatment of hepatitis B. Methods： Sixty patients with hepatitis B were randomly and equally divided into treatment group and control group. The control group received adefovir dipivoxil,while the treatment group received Wucao decoction in addition to adefovir dipivoxil. After one year of treatment,clinical outcome,the levels of alanine aminotransferase,aspartate aminotransferase,and total bilirubin and albumin / globulin ratio at different time points,and HBV DNA level were determined. Results： The treatment group had a significantly higher overall response rate than the control group（ 83. 3% vs66. 7%,P〈0. 05）. Both groups showed significant improvement in liver function after treatment（ P〈0. 05）,and the treatment group had significantly better liver function than the control group（ P〈0. 05）. HBV DNA load showed no significant difference between the two groups at months 3 and 6 of treatment,but the treatment group had a significantly higher HBV DNA clearance rate than the control group at months 9 and 12 of treatment（ P〈0. 05）.Conclusion： For patients with hepatitis B,Wucao decoction combined with adefovir dipivoxil has good clinical efficacy,can significantly improve patients＇ liver function,and increases HBV DNA clearance rate.
