甲泼尼龙及维生素K1治疗百日咳综合征的疗效观察

摘　　要：目的探讨甲泼尼龙及维生素K1治疗百日咳综合征的临床疗效及安全性。方法选择2014年6月至2015年6月就诊于郑州大学第一附属医院的百日咳综合征婴儿为研究对象，所有患儿首先给予大环内酯类抗感染及雾化吸入等基础治疗，入院后按数字表法将患儿分为A、B、C组三组，A组给予甲泼尼龙1mg／（kg·d），应用3～5d，B组给予维生素K1 1mg／（kg·d），应用3d，C组为对照组，分别比较三组患儿痉挛性咳嗽缓解时间、平均住院时间，观察治疗后患儿有无呼吸衰竭、烦躁、皮疹等不良反应。结果共38例患儿完成治疗疗程，A组12例，B组10例，C组16例，三组患儿的人口学特点、就诊前病程比较差异均未见统计学意义（P〉0．05），三组痉挛性咳嗽缓解时间及住院时间比较差异均有统计学意义（P〈0．05），其中A组痉挛性咳嗽缓解时间、平均住院时间与B组及C组比较差异均有统计学意义（P〈0．05），而B组与C组痉挛性咳嗽缓解时间及平均住院时间比较差异未见统计学意义（P〉0．05）。结论甲泼尼龙能有效改善百日咳综合征痉挛性咳嗽症状，缩短治疗疗程，但维生素K1疗效不明确。Objective To observe the treatment effects of methylprednisolone and vitamin K1 on pertussis syndrome. Methods Totally pertussis syndrome admitted to the first affiliated hospital of Zhengzhou university from June 2014 to June 2015 were enrolled. All the subjects were assigned to receive macrolide antibiotics and inhalation. Then the subjects were randomized to receive intravenous infusion of 2 -3 mg/kg methylprednisolone（ group A）, 1 mg/kg Vitamin K1 （ group B ） . The treatment lasted for 3 - 5 days. Group C was the no-treatment control group. The relief time of spasmodic cough and average stay were compared. Dyspnea, dysphoria and rash were recorded to assess safety. Results A total of 38 patients completed this research. Group A included 12 cases, group B included 10 cases, group C included 16 cases. Demographic characteristics, pre-treament duration before treatment were similar among the three groups. The relief time of spasmodic cough and average stay were significantly different among the three groups（ F = 15. 375, 6. 545 ,P 〈 0. 05 ） , and the symptom of group A were significantly less than that of the group B and group C （ P 〈 0. 05 ）, while group B and group C were similar. Conclusions Methylprednisolone can relieve clinical symptoms of pertussis syndrome and shorten treat- ment time, while Vitamin K1 is unknown.
