摘　　要：目的:探讨关节镜下保留残端腓骨长肌腱重建后交叉韧带临床效果。方法:2010年4月至2012年6月采用关节镜下保留残端自体腓骨长肌腱重建后交叉韧带28例,患者平均年龄36.7岁。术前Lysholm评分为(28.7±9.8)分,IKDC评分为(32.6±7.2)分。结果:术后切口均Ⅰ期愈合,无神经血管并发症发生。随诊时间平均28.3个月。术后患者胫骨后沉试验、后抽屉试验、后向Lachman试验均为阴性,膝关节不稳定症状消失。膝关节活动平均134.6°。术后Lysholm评分为(88.2±3.9)分,IKDC评分为(86.6±4.1)分,和术前比较,差异有统计学意义(P<0.01)。结论:关节镜下保留残端腓骨长肌腱重建后交叉韧带,取腱容易、安全,临床疗效满意,无明显并发症。Objective： To investigate the clinical outcome of the arthroscopic posterior cruciate ligament reconstruction with remnant preserving and peroneus longus tendon autograft. Methods： From April 2010 to June 2012, 28 cases of arthroscopic posterior cruciate ligament reconstruction with remnant preserving and peroneus longus tendon autograft, the average age was 36.7. The Lysholm knee score was（28.7±9.8） and the IKDC score was（32.6±7.2） before the operation. Results： All incisions healed by first intention, and there was no nerve and vascular injury. The average follow-up was 28.3 months. Posterior drawer test, posterior lachman test and posterior sag test were all negative, and knee instability symptoms disappeared. The average knee range of motion was 134.6°. The Lysholm knee score was（88.2±3.9） and the IKDC score was（86.6±4.1） after operation, which was statistically significant when compared with before the operation（P0.01）. Conclusion： Arthroscopic posterior cruciate ligament reconstruction with remnant preserving and peroneus longus tendon autograft, tendon harvesting is easy and safe, clinical outcome is satisfactory and no obvious complication.
