江西省孕产妇剖宫产影响因素分析

摘　　要：目的了解江西省剖宫产率现状及影响因素,为政府制定政策提供科学依据。方法采用随机整群抽样的方法抽取13家市、县级医疗保健机构的产妇为调查对象,调查产妇数、剖宫产数、产妇及丈夫信息。将剖宫产产妇与阴道顺产产妇分为剖宫产组和阴道产组,对比选择两种分娩方式的影响因素。结果在12 143例产妇中,4 752例行剖宫产术,剖宫产发生率为39.1%;三级医院的剖宫产率高于二级医院,妇幼保健专科医院的剖宫产率高于综合医院;多因素Logistic回归分析结果显示:产妇年龄、丈夫年龄、丈夫文化程度、家庭年人均收入、孕前BMI、丈夫职业、接受产前教育、既往病史、孕期合并症或并发症9个因素与剖宫产分娩有关。结论目前江西省剖宫产率较高,与剖宫产率升高相关的影响因素较多,应结合实际情况采取相应的干预措施以降低剖宫产率。Objective To understand the status and influencing factors of cesarean section in Jiangxi province,provide a scientific basis for the government to develop relevant policies. Methods The pregnant women were selected from 13 medical institutions at city and county level by random cluster sampling method,the number of pregnant women,the number of cesarean section,the situations of the pregnant women and their husbands were surveyed. The women were divided into cesarean section group and vaginal delivery group according to delivery modes,the influencing factors of the two delivery modes were compared. Results Among 12 143 women,4 752 women chose cesarean section,the rate was 39. 1%; the rates of cesarean section in tertiary hospitals were higher than those in secondary hospitals,the rates of cesarean section in maternal and child health care special hospitals were higher than those in general hospitals; multivariable logistic regression analysis showed that female age,male age,male educational level,per- capita annual income,progestational body mass index（ BMI）,male vocation,receiving prenatal education,medical history,and complications during pregnancy were correlated with cesarean section. Conclusion At present,cesarean section rate is rather high in Jiangxi province. There are many factors related to high cesarean section rate,and appropriate interventions should be adopted according to the actual situation to reduce cesarean section rate.
