摘　　要：目的探讨中性粒细胞-淋巴细胞比值（NLR）与胃癌预后的关系。方法检索建库至2015年3月在Pubmed、Embase和Science Direct等数据库公开发表的关于NLR与胃癌预后关系的文献，用Stata12．0软件进行统计学分析。结果16项符合标准的研究共计5774例胃癌患者纳入研究，其中2030例患者NLR升高。Meta分析结果显示，NLR升高是影响胃癌患者预后的危险因素，明显缩短胃癌患者的总体生存率（OS）[风险比（HR）=1．76，95％置信区间（CI）：1．59～1．96，P〈0．01）、无病生存期（DFS，HR=1．57，95％CI：1．12～2．20，P〈0．01）和无进展生存期（PFS，HR=1．61，95％CI：1．32～1．97，P〈0．01）。结论NLR升高的胃癌患者预后较差，临床上应重视NLR，使高风险胃癌患者及时得到合理治疗。Objective To evaluate the prognostic implication of neutrophil - to - lymphocyte ratio （NLR） in gastric cancer by meta - analysis. Methods Studies before March 2015 exploring prognostic imp｜icati^m of neutrophil - to - lymphocyte ratio in gastric cancer in the literature databases of PubMed, Embase and ScienceDircct were searched and analyzed by using Stata 12. 0. Results Sixteen studies which fitted criteria including 5 744 gastric cancer cases were involved in this metaanalysis. There were 2 030 patients with increased NLR. Meta - analysis showed the increased level of NLR was the risk prognostic factor of gastric cancer patients and indicated poorer overall survival （OS） [ hazard ratio （HR） = 1.76, 95% confidence interval （CI） ： 1. 59 - 1.96, P 〈 0. 01 ], disease - free survival （ DFS, HR = 1.57, 95%CI： 1.12-2.20,P〈0.01） and progression-free survival （PFS, HR= 1.61, 95%CI： 1.32 - 1.97, P 〈 0. 01 ）. Conclusion Elevated NLR was associated with a worse prognosis for patients with gastric cancer and attached great importance in clinical work to get rational treatment in time.
