摘  要:目的探讨右美托咪定对脑死亡大鼠肝脏损伤的保护作用及其机制。方法将30只无特定病原体(SPF)级的SD大鼠随机分为假手术组(Sham组)、脑死亡(BD组)和右美托咪定处理组(DEX组),每组10只。假手术组:仅进行手术操作,不进行脑死亡诱导;脑死亡组:采用缓慢颅内加骶法建立大鼠脑死亡模型;DEX组:在诱导大鼠脑死亡前通过尾静脉以5μg/(kg·h)的速度持续泵注右美托咪定1h,余同脑死f组。利用酶联免疫吸附试验(ELISA)法检测血清中谷丙转氨酶(ALT)、谷草转氨酶(AST)活性;Western blot检测肝脏组织中内质网应激相关因子葡萄糖调节蛋白78(Bip)、X盒结合蛋白-1(Xbp-1)、C/EBP环磷酸腺苷反应元件结合转录因子同源蛋白(CHOP)及半胱氨酰天冬氨酸特异性蛋白酶(Caspase)-12蛋白的表达;利用实时荧光定量核酸扩增检测系统(QPCR)检测CHOP及Caspase-12mRNA的表达。结果与Sham组(A明:22U/L;AST:39U/L)比较,BD组ALT(62U/L)、AST(84U/L)表达水平显著增加,肝脏组织中CHOP(相对值为3)及Caspase-12(相对值为2)mRNA表达增加,Bip、Xbp-1、CHOP及Caspase-12蛋白表达水平增加(积分吸光度值分别为:540.0±4.5、879.0±2.1、984.0±3.2),差异有统计学意义(P〈0.05);与BD组比较,ALT(43U/L)、AST(61U/L)表达水平降低,肝脏组织中CHOP(相对值为1.0)及Caspase-12(相对值为1.4)mRNA表达减少,Bip、Xbp-1、CHOP及Caspase-12蛋白表达水平减少(积分吸光度值分别为:210.0±3.1、431.0±1.5、516.0±1.7),差异有统计学意义(P〈0.05)。结论右美托定能减轻脑死亡状态下的肝脏损伤,其可能是通过内质网应激途径导致的凋亡途径来发挥作用的。Objective To determine the effects of dexmedetomidine (Dex) on liver injury in brain death rats. Methods Thirty SD rats were randomly divided into three groups (n = 10 per group): Sham group, brain death (total inflow occlusion for 30 min) group (BD) and Dex group [ Dex was intravenously infused and continuously at 5 μg/(kg·h) for 1 h before induction of brain death]. The samples of blood and hepatic tissue were taken after experiment. Serum alanine aminotransferase (ALT) and aspartate aminotransferase (AST) were detected by enzyme linked immunosorbent assay (ELISA). The mRNA expression of C/EBP homologous binding protein (CHOP) and Cysteinyl aspartate - specific protease (Caspase) - 12 were analyzed by real - time quantitative polymerase chain reaction detecting system (QPCR). Meanwhile, the protein expression of glucose - regulatedprotein78 ( Bip), X box - binding protein - 1 ( Xbp - 1 ), CHOP and Caspase - 12 was detected by Western blotting. Results There was significant difference between the BD group (ALT: 62 U/L; AST: 84 U/L) and Sham group (ALT: 22 U/L; AST: 39 U/L) in the levels of ALT and AST, the mRNA expression of CHOP (relative value 3 ) and Caspase - 12 (relative value 2), the protein expression of Bip, Xbp - 1, CHOP and Caspase - 12 [ absorbance (A) : 540. 0 +4. 5, 879. 0 +2. 1 , 984. 0 ±3.21 ( P 〈 0. 05 ). As compared with BD group, the serum ALT (43 U/L), AST (61 U/L), the mRNA expression of CHOP ( relative value 1 ) and Caspase - 12 ( relative value 1.4), the protein expression of Bip, Xbp - 1, CHOP and Caspase - 12 (A: 210. 0 ±3. 1, 431.0 ± 1.5, 516. 0 ± 1.7) were decreased in Dex group ( P 〈 0. 05). Conclusion Dex attenuates liver injury induced by brain death, partly through the inhibition of endoplasmic reticulum stress - induced apoptosis and liver injury.
