摘 要:目的探讨喂养方式对呼吸道感染患儿体液免疫功能及病原体感染率的影响。方法回顾性分析759名呼吸道感染患儿的临床资料,根据喂养方式分为3组:母乳喂养组、混合喂养组和人工喂养组,对比各组血清白蛋白、球蛋白水平及常见病菌感染率的差异。结果与人工喂养组比较,母乳喂养组11~12月龄婴儿球蛋白和7—8月龄婴儿免疫球蛋白G水平明显升高(P〈0.05)。不同喂养方式下,巨细胞、呼吸道合胞病毒、衣原体感染率比较差异无统计学意义,但对6个月内的婴儿,与混合喂养及人工喂养相比,母乳喂养有更低的EB病毒感染率(P〈0.05)及支原体感染率(P〈0.05)。结论母乳喂养有助于提升6个月内婴儿的免疫力,显著降低患儿EB病毒及支原体的感染率。母乳喂养对于4个月内婴儿球蛋白及免疫球蛋白水平增长并不明显,但在4~12个月时母乳喂养组球蛋白及免疫球蛋白水平显著增高。母乳可能通过某种机制促进婴儿自身球蛋白及免疫球蛋白的产生。Objective To investigate the effects of different feeding patterns on humoral immune function and pathogen infection rates for infants with respiratory tract infection. Methods Retrospective analysis was performed with the clinical data of 759 infants with respiratory tract infection. Infants were divided into breastfeeding group, mixed feeding group and artificial feeding group. The levels of serum albumin and globulin as well as common patho- gen infection rates were compared. Results Globulin level in infants 11 to 12 months old and immunoglobulin G level in infants 7 to 8 months old were significantly higher in breastfeeding group compared with those in artificial feeding group (P 〈 0.05 ). There was no significant difference in infection rates of cytomegalovirus, respiratory syncytial virus and chlamydia in different feeding patterns. However, infection rates of EB virus and mycoplasma were lower for infants under 6 months in breastfeeding group than those in mixed feeding group and artificial feeding group( P 〈 0.05 ). Con- dusion Breasffeeding is beneficial to improve immunity for infants under six months and reduce infection rates of EB virus and mycoplasma. Breastfeeding cannot significantly increase globulin and immunoglobulin in infants under 4 months, which significantly increase in infants between 4 months to 12 months. Breast milk may promote the produc- tion of infants" own globulin and immunoglobulin by certain mechanism.
