基于因子分析的北京市某县村卫生室发展模式研究

摘　　要：目的:通过因子分析法对影响农村医患信任的因素进行分析,从增强医患信任的角度为村卫生室的发展提出合理建议。方法:对北京市某郊区县14个乡镇的260所村卫生室及村医的基本情况、运行现状等进行问卷调查,结果:本研究从8个变量中提取了3个公因子,将村医的平均年龄、平均执业时间以及村医是同村人的比例3个变量纳为公因子1并将其命名为熟悉程度因子;将村卫生室被政府购买服务比例、卫生室平均药物种类2个变量归纳为公因子2并将其命名为服务可及性因子;将村卫生室平均设备种类、村医是执业助理医师比例、村医中专及以上学历比例3个变量归纳为公因子3并将其命名为服务专业性因子。3个公因子方差累计贡献率分别为33.189%、26.133%、19.398%。结论:从医患信任角度来看,村卫生室的发展模式需要从村医人选、村卫生室服务和所配药品以及新晋村医的医疗水平等方面进行改进。Objective： To reduce the dimensions of factors affecting the doctor- patient trust through factor analysis,and to provide reasonable suggestions for the development of village clinics. Methods： The basic situation,running status and so on of 260 clinics in 14 towns of a county in Beijing were surveyed and analyzed. Results： In this study,three common factors were extracted from eight variables,and three variables-the average age and the average time to practice medicine of village doctors,and the perception of the village doctors in all the villagers were satisfied as a common factor 1 named familiar extent factor; the village health proportion of government services purchased and the average drug clinics species were grouped into the common factor 2 named service availability factor; the average types of equipment in the clinics,the ratio of licensed assistant physician or physician and the proportion of village doctors having the secondary and higher education were grouped into common factor3 named professional service factor. The cumulative contribution rate of variance for three common factors were 33. 189%,26. 133% and19. 398%. Conclusion： From the perspective of the doctor- patient trust,the development of village clinic needs to be improved from the village doctor candidates and village health services that equipped with medicines and medical skill level and medicine level of coming village doctors and other aspects.
