摘　　要：以云南省禄丰县为例,对县级公立医院进行按疾病诊断相关组(DRGs)支付方式改革的具体做法、实施效果、存在的问题和原因进行分析。禄丰县公立医院住院次均费用低于云南省县级公立医院平均水平,是该县长期探索推行支付方式改革、控制住院费用过快增长的成果,实施DRGs支付方式改革对于缩短平均住院日、提高住院服务效率有一定作用。建立和推广符合县级公立医院实际情况的DRGs支付方式是一个循序渐进的过程,需要各级政府、各相关部门、医院和学者的共同努力。This paper took Lufeng County in Yunnan Province for an example,researched on implementation of diagnosis related groups（ DRGs） payment system in county- level public hospital,from specific practice and effect of implementation. The results showed that inpatient cost of Lufeng County public hospitals was lower than the average level of Yunnan Province. Exploring and implementing payment reform for a long time controlled hospitalization expenditure growth. The implementation of DRGs payment reform shortened average hospitalization days,and improved hospital service efficiency. Establishing and promoting DRGs payment system complied with the actual situation of county- level public hospitals is a gradual process,which needs the joint efforts of governments at all levels,relevant departments,hospitals,and scholars.
