通心络胶囊联合米力农治疗老年肺心病的临床观察

摘　　要：目的:观察通心络胶囊联合米力农治疗老年肺心病的临床疗效和安全性。方法:452例老年肺心病患者随机分为对照组及观察组。两组患者给予常规治疗,对照组在常规治疗的基础上给予口服通心络胶囊,2粒/次,3次/d,疗程均为30d。观察组在对照组治疗基础上生理盐水250m L+米力农10mg,以5~10mg·kg^(-1)·min^(-1)速度静滴,共治疗10d。观察两组治疗前后用力肺活量(FVC)、1s用力呼气容积(FEV_1)、肺动脉压力(PAP)、动脉血氧分压(PaO_2)、动脉血二氧化碳分压(PaCO_2)、白细胞介素(IL-6)、IL-8、肿瘤坏死因子-α(TNF-α)、内皮素-1(ET-1)和一氧化氮(NO)水平,记录不良反应发生情况。结果:治疗后,两组患者FVC、FEV_1、PaO_2、NO均显著高于治疗前,观察组高于对照组,差异均有统计学意义(P<0.01);两组患者PAP、Pa CO2、IL-6、IL-8、TNF-α、ET-1均显著低于治疗前,观察组低于对照组,差异均有统计学意义(P<0.01)。结论:通心络胶囊联合米力农治疗老年肺心病疗效和安全性均好,可降低肺血管性炎性反应及肺动脉高压,提高血氧饱和度,改善肺功能和心功能。Objective：To observe the efficacy and safety of Tongxinluo capsule combined with milrinone in the treatment of pulmonary heart disease.Methods：452 patients with pulmonary heart disease were randomly divided into control group and observation group. Two groups of patients were given conventional treatment. Control group was treated with Tongxinluo capsule for Oral 520 mg for continuous 1moths based on the treatment of conventional treatment,once a day. Observation group was treated with milrinone for intravenous drip10 mg for continuous 10 d based on the treatment of control group,once a day. The forced vital capacity（FVC）,forced expiratory volume（FEV_1）,pulmonary artery pressure（PAP）,arterial partial pressure of oxygen（PaO_2）,arterial partial pressure of carbon dioxide（PaCO_2）,interleukin-6（IL-6）,interleukin-8（IL-8）,tumor necrosis factor alpha（TNF-α）,endothelin-1（ET-1）,and nitric oxide（NO）were observed. The incidence of adverse reactions were recorded.Results：After treatment,the FVC,FEV1,PaO_2 and NO in 2group were significantly higher than before,and observation group was higher than control group（P0.01）. the PAP,PaCO_2,IL-6,IL-8,TNF-α and ET-1 in 2 group were significantly lower than before,and observation group was lower than control group（P0.01）.There was no significant difference incidence of adverse reactions（P 0.05）.Conclusions：Tongxinluo capsule combined with milrinone has good efficacy and safety in the treatment of pulmonary heart disease,it can reduce Pulmonary vascular inflammatory response and pulmonary artery hypertension,improve blood oxygensaturation,improve lung function and heart function.
