摘 要:目的分析扩张型心肌病的左室重构、整体应变和收缩功能的相关性。方法入选扩张型心肌病(DCM)患者25例,正常对照者30例。常规超声心动图测量左室舒张末期和收缩末期球形指数(SID、SIS)、射血分数(LVEF)、组织多普勒收缩期二尖瓣环峰值速度(sm),同时应用二维斑点追踪技术分析左室整体纵向应变(GSL)、径向应变(GSR)及圆周应变(GSC),并对参数间的相关性进行分析。结果与正常对照组相比,DCM组左室球形指数SID、SIS及LVEF、sm均明显减低,左室整体GSL、GSR和GSC均与LVEF、sm、SID、SIS呈高度负/正相关性,其中GSL是LVEF的独立预测因子。结论扩张型心肌病患者左室重构、左室心肌的变形能力、整体收缩功能,这三者间密不可分,GSL是LVEF的独立预测因子。Objective To analyze the relationships among the left ventricular remodeling,global strain,and systolic function in patients with dilated cardiomyopathy(DCM)using speckle tracking imaging and conventional echocardiography.Methods Twenty five patients with DCM and 30 healthy subjects were included.All the participants underwent transthoracic echocardiography.All images were stored digitally and analyzed off-line.Results There were a very significant differences in left ventricular systolic indices(LVEF,sm)and Sphericity indices(SID,SIS)between the two groups.Global longitudinal strain(GSL),global circumferential strain(GSC),and global radial strain(GSR)in patients with DCM were all different very significantly from the controls.There were strong correlations between the three global strain and LVEF,sm,SID,SIS.GSL was the independent predictor of LVEF.Conclusions The relationships among the deformation ability,LV remodeling,and global systolic function are inseparable.GSL is the independent predictor of LVEF.
