三维能量多普勒超声动态观察普萘洛尔治疗儿童浅表血管瘤的价值

摘　　要：目的探讨三维能量多普勒超声在动态观察血管瘤治疗中的意义。方法口服普萘洛尔治疗儿童浅表血管瘤56例,于治疗前、后1个月和3个月分别采集超声图像,建立数据库,比较治疗前后不同时段血管瘤的超声图像血管指数、血流指数及血管血流指数变化。结果口服普萘洛尔1个月:VI(0.46±0.08)、VFI(14.95±7.64)及FI(33.31±5.52)分别较治疗前VI(0.52±0.09)、VFI(19.54±7.37)及FI(37.01±5.63)下降(P<0.05),差异有统计学意义;治疗后3个月:VI(0.40±0.14)、VFI(13.76±5.00)及FI(32.87±3.78)与治疗前VI、VFI及FI比较,均下降明显(P<0.01),差异有显著性。结论三维能量多普勒超声在动态观察儿童血管瘤药物治疗过程中瘤体变化有重要价值。Objective To investigate the guiding significance of three-dimensional power doppler ultrasound in the dynamic observation of the treatment of hemangioma.Methods Fifty-six cases with superficial hemangioma were treated with propranolol.The ultrasonic images were obtained before one month of the treatment,and 3month after the treatment respectively.Then the database was established.Vascular vessels index,blood flow index and vascular blood flow index were compared.Results Oral propranolol 1 month：VI（0.46±0.08）,VFI（14.95±7.64）and FI（33.31±5.52）.Before treatment：VI（0.52±0.09）,VFI（19.54±7.37）and FI（37.01±5.63）.The former is higher,the difference was statistically significant（P〈0.05）.Three months later：VI（0.40±0.14）,VFI（13.76±5.00）and FI（32.87±3.78）,compared with former treatment,decreased significantly,the difference was statistically significant（P〈0.01）.Conclusions There is a great value of 3Dpower doppler ultrasound in observing the treatment change of children hemangioma.
