尿道板纵切卷管尿道成形术治疗小儿尿道下裂的疗效观察

摘　　要：目的探讨尿道板纵切卷管尿道成形术治疗小儿尿道下裂的临床疗效。方法选取我院2011年2月至2015年5月期间收治的280例小儿尿道下裂患儿作为研究对象,所有患儿阴茎屈曲在轻度以内,将所选患儿按治疗方式分为两组,每组各140例,对照组患儿采取尿道板纵切卷管尿道成形术(TIP法)进行治疗,研究组患儿行TIP治疗并加盖阴茎背侧筋膜瓣,比较两组患儿的临床治疗效果和并发症。结果研究组的治疗总有效率为93.57%,显著高于对照组的77.86%,差异有统计学意义(P<0.05)。研究组患儿的尿瘘率为0.71%,对照组为2.86%,组间差异无统计学意义(P>0.05)。研究组患儿的狭窄率为2.14%,显著低于对照组的10.71%,差异有统计学意义(P<0.05)。结论适当进行改良的尿道板纵切卷管尿道成形术可作为小儿尿道下裂的首选治疗方案,可提高患儿的临床治疗效果,降低并发症发生率。Objective To explore the clinical curative effect of tubularized incised-plate urethroplasty for the treatment of children with hypospadias. Methods A total of 280 cases of children with hypospadias admitted to our hospital from February 2011 to May 2015 were selected and divided into two groups according to the treatment methods. 140 cases of control group received tubularized incised-plate urethroplasty, while 140 cases of the study group received tubularized incised-plate urethroplasty and stamped with dorsal penile fascia. The curative effects and complications of two groups were compared. Results The total effective rate of study group was 93.57%, significantly higher than 77.86% of control group, with statistical difference（P〈0.05）. The incidence rate of fistula was 0.71% of study group, and 2.86%of control group, with no statistical difference（P〉0.05）. The incidence rate of narrow of study group was 2.14%, significantly lower than10.71% of control group, with statistical difference（P〈0.05）. Conclusions Appropriately modified tubularized incised-plate urethroplasty can be considered as the primary treatment for children with hypospadias, which can improve the curative effect, and redeuce the incidence of complications.
