腹腔镜精索静脉高选择性结扎术与集束结扎术疗效比较

摘　　要：目的：比较腹腔镜精索静脉高选择结扎术及腹腔镜精索静脉集束结扎术的疗效。方法：选取我院2012年1月~2014年6月因左侧精索静脉曲张手术患者120例,其中60例行腹腔镜精索静脉高选择性结扎术（观察组）,60例行腹腔镜精索静脉集束结扎术（对照组）。比较两组患者手术情况,包括手术时间、出血量、术后住院时间,术后随访6个月,比较术后患者症状改善情况、术后精液质量改善情况,并比较术后复发及并发症（睾丸萎缩、鞘膜积液）情况。结果：120例患者手术均获成功,两组在手术时间、术中出血量及术后并发症的比较差异有统计学意义,术后平均住院时间、术后复发率、术后症状改善情况及术后精液改善情况比较差异无统计学意义。结论：两种手术方式对精索静脉曲张疗效均能取得良好的治疗效果,但腹腔镜精索静脉高选择性结扎术在防止术后并发症的发生方面更有优势。Objective： To compare the effects between laparoscopic high-selectivity spermatic vein ligation and laparoscopic cluster spermatic vein ligation. Method： From January 2012 to June 2014, 120 patients with left vari- cocele were divided into observer group （60 patients who underwent laparoscopic high-selectivity spermatic vein ligation） and control group （60 patients who underwent laparoscopic cluster spermatic vein ligation）. The opera- tion time, the blood loss and the mean postoperative hospital stay were analyzed. All the patients were followed up for six months. The improvement of symptoms and the improvement of semen quality were analyzed, and the re- currence and complications （testicular atrophy, hydrocele） were also analyzed. Result： All operations were suc- cessful. The analysis about operative time, blood loss and complications were statistically significant between the two groups. However, the mean postoperative hospital stay, recurrence rate, postoperative improvement of symptoms and postoperative semen improvement were not statistically significant. Conclusion： Both the two kinds of surgical approches to varicocelectomy are effective, but the complication rate of laparoscopic high-selectivity spermatic vein ligation is lower than the other one.
