宫腔球囊填塞术与宫腔塞纱术治疗剖宫产产后出血疗效分析

摘　　要：目的探讨、比较宫腔填塞球囊与宫腔塞纱治疗剖宫产产后出血的效果。方法回顾性分析2013年1月至2015年3月在本院剖宫产分娩产妇的分娩资料，选择发生产后出血的产妇作为研究对象，共41例。以应用Bakfi子宫球囊填塞术治疗的23例为研究组，应用宫腔塞纱术治疗的17例为对照组。其中1例因先使用宫腔塞纱后改为宫腔填塞球囊治疗，最后切除子宫予以剔除。对两组一般情况、高危因素、治疗效果、预后情况等进行对比分析。结果研究组产后2h增加出血量（98．430±132．066）ml，24h增加出血量（212．520±203．094）ml；对照组产后2h增加出血量（194．240±155．360）ml，24h增加出血量（432．820±194．346）ml；差异均有统计学意义（P〈0．05）。两组产前产后24h血红蛋白差值、产前产后24h血红蛋白下降率、输血量、术后24h白细胞计数、住院天数差异无统计学意义（P〉0．05）。无一例行血管结扎术或急诊子宫切除术。结论在治疗剖宫产产后出血中，宫腔球囊填塞术对于减少术后2h出血和术后24h出血显著优于宫腔塞纱术，操作简单，值得推广。Objective To compare the effectiveness of intrauterine balloon tamponade with that of intrauterine yarn plugging in the treatment of hemorrhage after cesarean section. Methods The delivery records of the puerperae undergoing cesarean section at the obstetrical department of our hospital from January, 2013 to March, 2015 were retrospectively reviewed. The women who underwent cesarean delivery and occurred postpartum hemorrhage were selected as study objects. 41 cases were collected. 23 cases treated with intrauterine balloon tamponade were categorized as a study group and 17 treated with intrauterine yam plugging a control group. 1 case treated with two methods and finally removed her uterine was excluded. The general conditions, high risk factors, treatment effect, and prognosis of the two groups were compared and analyzed. Results The increments of hemorrhage 2 and 24 hours after delivery were （98.430±132.066） ml and （212.520±203.094） ml in the study group and were （194.240±155.360） ml and （432.820±194.346） ml in the control group, with statistical differences （P〈0.05）. There were no statistical differences in hemoglobin decline rate, blood transfusion volume, leukocyte count 24 hours after cesarean section, and hospital stay between these two groups （P〉0.05）, No case underwent vessel ligation or hysterectomy in both groups. Conclusions Intrauterine balloon tamponade in the treatment of postpartum hemorrhage is better in decreasing hemorrhage 2 and 24 hours after cesarean section than intrauterine yam plugging and is simple and is worth being generalized.
