青春期多囊卵巢综合征患者代谢综合征的患病特点

摘　　要：目的探索青春期多囊卵巢综合征（PCOS）患者代谢综合征（MS）患病特点。方法收集80例青春期PCOS患者MS患病率与肥胖、年龄、胰岛素抵抗和ApoB／ApoA1比值的关系。结果青春期PCOS患者MS患病率为12．50％，其中非肥胖患者MS和前代谢综合征阶段（pre—MS）患病率分别为0．00％和7．55％，低于肥胖患者的3704％和33．33％；两组pre—MS患病率差异有统计学意义（P〈0．05）。13～15岁组MS患病率为6．67％，低于16～19岁组的13．85％（P〈0．05）；pre—MS患病率为2000％，高于16～19岁组的15．38％.胰岛素抵抗型患者的MS和pre—MS患病率分别为3333％和33．33％，高于非胰岛素抵抗型患者的6．45％和1129％（P〈0．05），ApoB／ApoA1≥0．6的患者MS和pre—MS患病率分别为3333％和16．67％，高于ApoB／ApoA1〈06的患者645％和16．13％（P〈0．05）。结论青春期PCOS患者的MS患病率以肥胖、胰岛素抵抗型、ApoB／ApoA1≥06者较高。其中13～15岁人群pre—MS患病率较高。Objective To assess the features of metabolic syndrome （MS） in adolescent patients with polycystic ovary syndrome （PCOS）. Methods The prevalence and features of metabolic syndrome in 80 adolescent patients with PCOS were analyzed. Results Among 80 adolescent PCOS patients, metabolic syndrome was diagnosed in 10 cases with a prevalence rate of 12.50%, The prevalence of MS and pre-MS in obese patients was 37.04% and 33.33%, and 0.00% and 7.55% in non-obese patients, respectively （P〈0.05）. The prevalence of MS in 13-15 age group was 6.67%, and 13.85% in 16-19 age group, respectively（P〈0.05）. The prevalence of pre-MS in 13-15 age group was 20.00%, and 15.38% in 16-19 age group（P〈0.05）. The prevalence of MS and pre-MS in insulin resistance（ IR ）group was both 33.33%, that was 6.45% and 11.29% in non-IR group（P〈 0.05）. The prevalence of MS and pre-MS in patients with ApoB/ApoA1≥0.6 was 33.33% and 16.67%; while that was 6.45% and 16,13% in those with ApoB/ApoA1 〈0.6 （P〈0.05）. Conclusion The prevalence of metabolic syndrome in adolescent PCOS patients is increased with the age, and the obese adolescent PCOS women with IR and ApoB/ApoA1 ≥0.6 tend to have higher prevalence of metabolic syndrome.
