摘 要:目的探讨超声实时心肌造影对X综合征心肌微循环定量评价的应用价值。方法选取心脏X综合征患者78例(观察组)和78例健康志愿者(对照组);常规超声心动图记录反映心脏收缩功能的舒张末期左心室内径(LVEDD)、收缩末期左心室内径(LVESD)、左心室射血分数(LVEF)、收缩期二尖瓣环运动速度(S')、舒张末期A峰血流速度(A)、舒张末期二尖瓣环运动速度(A')以及反映左心室舒张功能的二尖瓣口舒张期E峰血流速度(E)、舒张期二尖瓣环运动速度(E')、E/A值、E波速度下降时间(EDT)、E/E'值。实时心肌超声造影检查记录注入造影剂后心肌微循环灌注开始时间(AT)、达峰时间(APT)、造影剂灌注的峰值强度(PI)及灌注速度(β),并计算A×β。结果两组LVEDD、LVESD、LVEF、A、S'、A'比较无统计学差异(P均>0.05);与对照组比较,观察组E值、E/A值、E'峰速值减小,EDT、AT、APT延长,E/E'增大,差异有统计学意义(P均<0.05);观察组造影剂灌注PI和β均低于对照组,A×β值小于对照组,差异有统计学意义(P均<0.05)。结论作为非创伤性观察心肌及心内膜下微循环灌注的新方法,实时心肌超声造影对X综合征心肌微循环的定量评价具有较高的临床应用价值。Objective To investigate the application value of real-time myocardial contrast echocardiography( MCE)in the quantitative evaluation of myocardial microcirculation of X syndrome. Methods Seventy-eight cases of patients with X syndrome( observation group) and 78 healthy volunteers( control group) were selected. We used the conventional echocardiography to record the left ventricular end-diastolic diameter( LVEDD), left ventricular end-systolic diameter( LVESD),left ventricular ejection fraction( LVEF),systolic mitral valve ring motion velocity( S'),end-diastolic blood flow peak velocity value( A),the late diastolic mitral valve ring motion velocity( A') and to record mitral diastolic blood flow peak velocity value( E),E/A value,diastolic mitral valve ring motion velocity( E'),E wave speed down time( EDT) and E/E' value of the two groups. Then,we used real-time MCE to record the perfusion start time of myocardial microcirculation after injection of contrast agents( AT),time to peak( APT),peak intensity of the contrast agent perfusion( PI) and perfusion rate( β),and to count A × β. Results No statistical difference was found in LVEDD,LVESD,LVEF,A,S',A' between the observation group and control group( all P 0. 05). Compared with the control group,E value,E/A value and E 'peak velocity value decreased,EDT lengthened,E/E' value increased,and AT,APT time prolonged in the observation group( all P 0. 05). PI of contrast agent perfusion and reperfusion infusion speed β of the observation group were lower than those of the control group,and the A × β value was less than that of the control group( all P 0. 05). Conclusion Real-time MCE as a non-traumatic and new method of observing myocardial and endocardial microcirculation perfusion,has a high clinical theoretical significance and practical application value in the quantitative evaluation of myocardial microcirculation of X syndrome.
