摘 要:目的:探讨TB/HIV双重感染(以下简称双感)患者的心理健康状况及其与社会支持情况的关系,为提高双感患者心理健康水平提供科学依据。方法:将安徽省双感率较高的7个县区可随访到的134例双感患者纳入研究,同时随机抽取134例结核病患者作为对照组,采用UCLA孤独量表、自评抑郁量表(SDS)及社会支持评定量表(SSRS)等进行问卷调查。结果:双感患者的社会支持总分、客观支持分、主观支持分及对支持利用度与抑郁和孤独得分之间呈显著负相关(P<0.05);双感组与结核病组在抑郁严重指数方面差异具有统计学意义(P<0.05),抑郁的发生率亦高于结核病组(P<0.05)。结核病患者的主观支持得分高于双感患者(P<0.05);在社会支持总分和客观支持方面两组差异无统计学意义(P>0.05),TB/HIV组的孤独得分(46.29±9.74)和抑郁得分(46.42±8.09)均高于TB组的孤独得分(39.91±8.32)和抑郁得分(39.53±7.60),P<0.05。结论:与结核病患者相比,双感患者心理健康问题较严重,因此,医疗卫生机构应采取有针对性的健康教育及心理疏导措施,努力提高双感患者的心理健康水平。Objective To explore the mental health status of patients with TB/HIV double infection and its relationship with social support,and to provide scientific evidence for improving mental health.Methods A total of 134 patients with TB/HIV double infection were included in the study,and 134 cases of TB patients were randomly selected as control group.A questionnaire survey was conducted among these patients with the UCLA loneliness scale,self rating depression scale(SDS)and social support rating scale(SSRS).Results There was significantly negative correlation among social support,objective support,subjective support,support utilization degree and depression,loneliness score of the patients with TB/HIV double infection(P〈0.05).Statistical significance was found in depression severity index between the two groups,and the incidence of depression was higher in TB/HIV group(P〈0.05).The subjective support score of TB patients was significantly higher than patients with TB/HIV double infection(P〈0.05).The loneliness score and depression score of TB/HIV group were higher than TB group(P〈0.05).Conclusions The mental health problem of patients with TB/HIV double infection was more serious than TB patients,therefore,medical institutions should take targeted measures of health education and psychological counseling to improve the level of mental health among patients with TB/HIV double infection.
