摘 要:[目的]观察半夏泻心汤对大鼠肺胃组织P物质表达的干预作用,探讨肺胃相关理论的神经免疫学机制。[方法]将32只雄性SD大鼠,按随机数字表法分为正常对照组、模型对照组、半夏泻心汤组和雷尼替丁组。采用慢性食管内灌注盐酸方法建立大鼠食管、胃黏膜慢性炎症模型,半夏泻心汤组和雷尼替丁组分别予半夏泻心汤5.4g·kg-1灌胃和雷尼替丁27mg·kg-1灌胃,正常对照组及模型对照组予等量生理盐水灌胃,实验结束后取食管、胃和肺组织标本进行病理学观察和P物质免疫组化法测定,并对实验数据进行统计学分析。[结果]与正常对照组比,模型对照组肺、食管及胃黏膜炎症病理积分均显著升高,差异有统计学意义(P<0.01),肺、胃组织中P物质表达均显著增多,差异有统计学意义(P<0.01),各组大鼠食管组织P物质表达差异无统计学意义(P=0.05)。与模型对照组比,半夏泻心汤组大鼠肺炎症积分显著降低,胃及肺组织中P物质表达显著减少,差异均有统计学意义(P<0.01),但两组大鼠食管及胃黏膜炎症病理积分差异无统计学意义(P>0.05)。半夏泻心汤组与雷尼替丁组比较,肺、食管和胃黏膜炎症病理积分,肺、食管和胃组织P物质表达差异均无统计学意义(P>0.05)。大鼠肺组织和胃组织的P物质表达呈正相关(r=0.660)。[结论]慢性食管内灌注盐酸可以诱导大鼠食管-支气管反射通路发生P物质表达改变,提示肺胃P物质相关性表达可能是中医肺胃相关理论的神经免疫机制之一,半夏泻心汤可能通过P物质干预其神经免疫机制。[Objective] To observe the intervention effect of Banxia Xiexin decoction on substance P(SP) of lung and stomach in rats, and investigate neuro- immune mechanism of lung and stomach theory. [Methods]32 male SD rats were randomized into 4 groups:blank control group,model group,Banxia Xiexin decoction group and ranitidine group.The animal model was established by using of repeated intra-esophageal instillation of HC1. Then,the treatment was given for 7 days.The expressions of SP in the lung, esophagus and stomach were detected by immunohistochemistry.[Results] Pathological integral of lung, esophagus and stomach in model group were significantly higher than those in blank control group(P〈0.01).The expressions of SP in the lung and stomach in model group were significantly higher than those in blank control group(P〈0.01). Compared with the model group,the Banxia Xiexin decoction group reduced the inflamraatory score of lung and expressions of SP in the lung and stomach significantly(P〈O.O1). There was no statistical significance among the P substance expressions of lung, esophagus and stomach tissue.There was a positive correlation between lung and stomach with expression of SP(r=O.660).[Conclusion] Repeated intra-esophageal instillation of HC1 can induce SP alternations in esophageal-bronchial reflex pathway in rats,which indicated that expression of SP would be one of neuro-immune mechanisms of lung and stomach theory. Banxia Xiexin decoction would interfere in neuro-immune mechanism of lung and stomach theory by SP.
