摘  要:目的探讨扩散加权成像(DWI)鉴别三脑室良恶性病变的价值。方法收集我院59例经手术病理证实的三脑室病变进行分析。测量病变表观扩散系数(ADC)值,绘制ROC曲线并获得鉴别肿瘤良恶性的阶段点及其诊断的敏感性及特异性。结果恶性肿瘤平均ADC值(1.03±0.16)×10^(-3) mm^2/s显著低于良性病变(1.83±0.95)×10^(-3) mm^2/s。以1.19×10^(-3) mm^2/s为阈值诊断三脑室良恶性病变的敏感性、特异性分别为94.1%、95.5%。而且,畸胎瘤(1.35±0.05)×10^(-3) mm^2/s、混合性生殖细胞瘤分别与其他生殖细胞瘤(0.73±0.12)×10^(-3) mm^2/s差异有统计学意义,高级别胶质瘤(1.04±0.03)×10^(-3) mm^2/s、低级别胶质瘤(1.32±0.11)×10^(-3) mm^2/s之间统计学也有差异。结论 DWI-ADC在鉴别三脑室良恶性病变方面有较高价值及较好的临床实用性。Objective To explore the ability of DWI in diagnosis of benign and malignant tumor or tumor mimics of the third ventricle. Methods MRI and DWI data of 59 pantients with the third ventricle tumors or tumor mimics proved by pathological were retrospectively analyzed.In DWI,ADC was measured and the ROC was drawn to obtain the cut-off value of ADC for differetial diagnosis of malignant tumors. Rusults The mean ADC vlaue of malignant tumors[(1.03±0.16)×10^(-3)mm^2/s] was significantly lower than bengin tumors and tumor mimics [(1.83±0.95)×10^(-3)mm^2/s]. Taking 1.19×10^(-3)mm^2/s as cut-off value,the sensitivity,specificity in diagnosing malignant tumors using ADC was 94.1% 、95.5%. And,the ADC value of teratoma [(1.35 ±0.05) ×10^(-3)mm^2/s] and mixed germ cell tumors[(1.20±0.07)×10^(-3)mm^2/s]、low grade glioma [(1.35±0.07)×10^(-3)mm^2/s] and high grade gliomas [(1.04±0.03)×10^(-3)mm^2/s] also have statistical significance respectively. Conclusion The ability of DWI-ADC vlaue in differentiating benign and malignant tumor or tumor mimics of the third ventricle is high,and has pratical application value.
