单孔全胸腔镜肺叶切除术30例临床体会

摘　　要：目的探讨单孔式全胸腔镜肺叶切除术的安全性和可操作性。方法对哈尔滨医科大学附属第二医院胸外科2014年7月~2015年1月间30例全胸腔镜肺叶切除病例资料进行回顾性分析。结果30例患者均于全胸腔镜下完成单孔式肺叶切除术,无中转开胸病例,其中2例患者因切缘原因,行左全肺切除。术后病理非小细胞肺癌19例,支气管扩张症5例,中叶综合征3例,小细胞肺癌2例,肺良性肿瘤1例。患者平均手术时间为（115±60）min（20~195 min）,术中平均失血量为（100±72）m L（20~200 m L）,平均胸腔带管时间为（3.2±1.6）d（3~5 d）。30例患者无围手术期死亡患者,无支气管胸膜瘘等术后严重并发症,均顺利出院。结论单孔式全胸腔镜下肺叶切除手术是一种安全、可操作的微创手术方式,值得在临床推广应用。Objective To study the safety and operability of uniportal video-asisted thoracoscopic lobectomy. Methods Thirty patients with lung diseases were underwent uniportal video-asisted thoracoscopic lobectomy from July 2014 to January 2015,and the clinical data were retrospectively analyzed. Results The uniportal video-asisted thoracoscopic lobectomy was performed in all cases. There was no case converted into thoracotomy. The mean operative time was（ 115 ± 60） min,the mean intraoperative blood los was（ 100 ± 72） m L,the mean thoracic drainage time was（ 3. 2 ± 1. 6） d. All patients were discharged without perioperative complications and postoperative death in 30 d. Conclusion The uniportal video-asisted thoracoscopic lobectomy is a safe and operable method of minimaly invasive technique,and it is worthy to apply in thoracic surgery.
