耳穴压豆结合中频脉冲电治疗促进剖宫产术后患者泌乳的临床观察

摘　　要：【目的】观察耳穴压豆结合中频脉冲电治疗促进剖宫产术后患者泌乳的临床疗效。【方法】将240例剖宫产术后患者随机分为治疗组与对照组各120例。对照组给予产后常规母乳喂养指导及乳房护理,治疗组在产后常规母乳喂养指导及乳房护理的基础上给予耳穴压豆结合中频脉冲电治疗。观察2组临床疗效、乳房充盈度、补授乳量及血清催乳素(PRL)水平变化情况。【结果】(1)治疗组总有效率为90.0%,显著优于对照组的67.5%,差异有统计学意义(P<0.05)。(2)治疗后,治疗组血清PRL水平较治疗前显著上升(P<0.05),而对照组升高不明显(P>0.05),治疗组血清PRL水平较对照组显著升高(P<0.05)。(3)治疗后,治疗组的乳房充盈度较对照组显著增加,补授乳量较对照组显著减少,差异有统计学意义(P<0.05或P<0.01)。(4)治疗期间,2组产妇及婴儿均无明显副作用及不良反应发生。【结论】耳穴压豆结合中频脉冲电治疗能有效促进剖宫产术后患者乳汁分泌,提高母乳喂养率,其作用机制可能在于调节催乳素的分泌。Objective To observe the clinical effect of sticking-pressure of Semen Vaccariae（SV） on auricular points combined with intermediate frequency electric pulse treatment on promoting lactation of women after cesarean section. Methods A total of 240 women after cesarean section were randomly divided into treatment group and control group, 120 cases in each group. Both groups were given routine puerperal breast-feeding instruction and breast care, and additionally, the treatment group was given auricular point SV sticking-pressure combined with intermediate frequency electric pulse treatment. Therapeutic effect of both groups was evaluated,and the changes of breast engorgement,amount of milk substitute and serum prolactin（PRL）before and after treatment were observed. Results（1）The overall effective rate of treatment group was 90.0%,and that of the control group was 67.5%, the difference being statistically significant in two groups（P〈0.05）.（2）Serum PRL level of the treatment group was increased after treatment（P〈0.05 compared with that before treatment）, but there was no obvious change in PRL level of the control group（P〉0.05）. The treatment group had higher PRL level than the control group（P〈0.05）.（3） After treatment, breast engorgement of the treatment group was increased while the amount of milk substitute was reduced as compared with those of the control group,and the differences were statistically significant（P〈0.05 or P〈0.01）.（4）No side effect or adverse reaction was found in the infant and parturient. Conclusion Auricular point SV sticking-pressure combined with intermediate frequency electric pulse treatment can effectively promote lactation of women after cesarean section and increase the rate of breast feeding. The possible mechanism may be related with the regulation of prolactin secretion.
